COVID-19疫情對於非緊急手術與處置醫療利用之影響

摘要


研究背景:2019年底於全球開始擴散的COVID-19疫情,造成世界各地的醫療服務產生巨大的衝擊。然而,距今約20年前,我國也曾爆發全球性的傳染病疫情,當時(2002~2003年) 嚴重急性呼吸道症候群(severe acute respiratory syndrome, SARS)疫情期間的醫療利用,有大幅減少的情形。 研究目的:為了瞭解民眾是否會因對COVID-19疫情的恐懼,而影響其就醫行為,延遲一些非緊急手術與處置行為,本研究分析COVID-19疫情爆發後,病人於醫療院所執行非緊急手術及處置之等候時間是否有變化。 研究方法:使用統計軟體為SAS Enterprise Guide 7.1版,針對健保資料庫收載2019年1月1日至2020年12月31日期間之門、住診申報資料進行分析,並使用描述性統計與多變項統計等統計方法,分析COVID-19疫情前後非緊急手術與處置等候時間之變化。 研究結果:相較於COVID-19疫情爆發前(2019年),2020年病人接受非緊急手術及處置之等候時間多為減少的情形,但進一步分析2020年上下半年的資料,2020年1~6月病人接受非緊急手術及處置之等候時間呈現增加的現象。 結論:COVID-19疫情爆發後,有延遲病人接受非緊急手術及處置之等候時間,顯示疫情可能會影響民眾就醫意願。

Background: The COVID-19 pandemic, which began to spread globally at the end of 2019, has tremendously impacted the utilization of medical services worldwide. In a similar situation about twenty years ago in Taiwan, the utilization of medical services during the period of severe acute respiratory syndrome (SARS, 2002 to 2003) significantly reduced. Objective: In order to investigate whether the fear of COVID-19 could influence the behavior of patients seeking medical services and thus delay non-urgent surgery and treatment, this study assessed changes in waiting times for non-urgent surgery and treatment during the outbreak of the COVID-19 pandemic. Methods: SAS Enterprise Guide version 7.1 was used to analyze outpatient and inpatient data extracted from the National Health Insurance research databases from January 1st, 2019, to December 31st, 2020. Descriptive statistics and multivariate statistical analysis were performed. Results: Waiting times for non-urgent surgery and treatment in 2020 significantly reduced compared with waiting times in 2019. Interestingly, the waiting times for non-urgent surgery and treatment from January to June 2020 were longer than the waiting times from July to December 2020. Conclusions: The COVID-19 pandemic indeed influenced the behavior of patients seeking medical services, which resulted in a delay of non-urgent surgery and treatment.
