PERP 基因多型性與口腔癌細胞分化相關性探討

Study of PERP gene polymorphism associated with cell differentiation in oral cancer

摘要


台灣口腔癌(oral cancer)是男性死亡率排行第四名的癌症。PERP (p53 apoptosis effect related to PMP-22)是抑癌蛋白p53的下游基因,與p53執行細胞凋亡(apoptosis)的路徑有關,並轉譯形成橋粒蛋白(desmosomal)。PERP基因多型性(gene polymorphism)與口腔癌的關係還不清楚。本篇目的是探討PERP基因多型性與口腔癌細胞分化(cell differentiation)程度的關聯性。我們收集了741位口腔癌患者,分析了口腔癌相關臨床參數,包括臨床分期、腫瘤大小、淋巴結轉移、遠端轉移等,同時也利用即時定量聚合酶連鎖反應 (real-time polymerase chain reaction)分析PERP rs651662、rs648802及rs2484067基因多型性。分析結果發現,細胞分化程度中等或差有較高的臨床分期(III/IV期)(p=0.009)與較高比例的淋巴結轉移(p =0.001)。口腔癌患者PERP rs651662 A/A基因型(genotype)比G/G野生型有3.517倍的機率口腔癌細胞分化程度較好,G/A+A/A基因型相較於G/G基因型有1.808倍的機率口腔癌細胞分化程度較好;口腔癌患者PERP rs648802,G/G基因型相較於C/C基因型有3.521倍的機率口腔癌細胞分化程度較好,C/G+G/G基因型相較於C/C基因型有1.775倍的機率口腔癌細胞分化程度較好,而PERP rs2484067基因多型性與口腔癌細胞分化程度無直接的關係。此外,我們也發現PERP rs651662等位基因(allele)為G時,口腔癌細胞分化程度較差(OR=1.736, p<0.001)。PERP rs648802等位基因為C時,口腔癌細胞分化程度較差(OR=1.716,p<0.001)。PERP rs2484067等位基因為C相較於等位基因為T,在統計學上無顯著差異(p =0.230)。綜合以上的研究結果,PERP rs651662與rs648802 基因多型性有助於預測口腔癌細胞分化程度,可能可以當作口腔癌發展的預測因子。

In Taiwan, oral cancer is the fourth cause of cancer death in men. PERP (p53 apoptosis effect related to PMP-22), a downstream target of tumor suppressor protein p53, is related to p53-dependent apoptosis and encoding a desmosomal protein. PERP gene polymorphism in oral cancer is poorly understood. The purpose of this study was to access the correlation between PERP gene polymorphism and cell differentiation in oral cancer. We collected 741 patients with oral cancer, analyzed the clinical parameters including clinical stage, tumor size, lymph node metastasis , distant metastasis and PERP rs651662、rs648802 and rs2484067 genetic polymorphisms were analyzed by real-time polymerase chain reaction(real-time PCR).Our results showed that moderate or poor cell differentiation are correlated with higher clinical stage III/IV(p=0.009)and higher lymph node metastasis rate(p =0.001). Oral cancer patients with A/A genotype of PERP rs651662 were at a significant 3.517-fold higher rate of well cell differentiation when compared to individuals with G/G wild-type, and that with G/A+A/A genotype were at a significant 1.808-fold higher rate of well cell differentiation when compared to individuals with G/G genotype;oral cancer patients with G/G genotype of PERP rs648802 G/G genotype were at a significant 3.521-fold higher rate of well cell differentiation when compared to individuals with C/C genotype, and that with C/G+G/G genotype were at a significant 1.775-fold higher rate of well cell differentiation when compared to individuals with C/C genotype. PERP rs2484067 gene polymorphism is less corrected with oral cancer cell differentiation. Moreover, we also found G allele of PERP rs651662 related to poor cell differentiation (OR=1.736, p<0.001). C allele of PERP rs648802 related to poor cell differentiation(OR=1.716, p<0.001). C allele of PERP rs2484067 compared with T allele , there was no significant difference of cell differentiation (p<0.230).In conclusion, PERP rs651662 and rs648802 polymorphism has potential predictive significance in oral cancer cell differentiation and may be as factors to predict the development in oral cancer.
