架構於無所不在網路技術的行動照護系統

摘要


根據行政院經濟建設委員會推估,2018年我國高齡人口將達14%,是聯合國世界衛生組織所定義的高齡社會國家。 高齡者通常都有慢性病纏身,根據內政部老人生活狀況調查及美國退休人士協會的研究顯示,高齡族群面臨醫療照護需求時,約有八成的人都偏好尋求在家接受醫療照護服務,顯示居家醫療照護已經成為全球的重要趨勢。 遠距照護系統可以提供長期照護服務的功能,收集病人生理參數以供醫師診斷,同時亦縮短調整診斷處方的時間,使照護人員充份掌握病患的生理狀況。 本論文引入遠距照護之概念,以無線網路、網際網路及手機通訊等無所不在網路的技術,應用於遠距病患生理監視,結合居家照護與醫院資源,以建構遠距醫療照護監視資訊系統,完成一個整合多種生理參數擷取裝置的行動照護系統,其中將生理參數擷取裝置,和作為個人行動閘道器的手機分開設計,並各自建構。照護中心建構警報和管理機制,使照護中心可以隨時隨地主動的提供照護服務,我們實現了一個照護平台的雛形,實驗的結果顯示,我們所提出的系統架構確實可提供遠距及行動照護的服務,而具有實用性。

According to the population projection reported by the Council for Economic Planning and Development, Executive Yuan, the projected proportion of the old-age population in Taiwan will reach 14% by 2018 which is far beyond the definition of aged society by the WHO. The aged people are usually held up by chronic diseases. The survey of senior citizens condition conducted by Ministry of the Interior and the study of the American Association of Retired Persons (AARP) reveal that approximately 80% of the old-age population prefer to request in-home medical care service when they need medical care. Therefore, medical home care has become an important trend in the world. A tele-medical care system can provide a long-term care service which collects the patient's physiological parameters for physician's diagnosis. It not only can reduce the time of diagnosis and treatment but also enables health care providers to fully control the patient's physiological condition. This dissertation introduces the concept of tele-medical care to the physiological monitoring for remote patients by the technology of ubiquitous network, such as wireless network, Internet and mobile communications. It combines home healthcare and hospital's resources to construct a tele-medical monitoring information system which is a mobile care system integrated with a multiple-physiological-signal acquisition device. The physiological data acquisition device and the handset used as the personal mobile gateway are designed and constructed separately. The care center sets up an alarm and management mechanism so that the care center can actively provide medical care service whenever and wherever possible. We have implemented a prototype of healthcare platform and the experimental results demonstrate the proposed system architecture is indeed practicable to provide mobile tele-medical care service.
