社區醫療群合作夥伴選擇模式之研究

摘要

為了落實分級醫療與轉診制度，健保局已於2003年便推出「全民健康保險家庭醫師整合性照護制度試辦計畫」，是一種針對醫療費用控制與品質提升之要求，而產生的整合性醫療照顧體系 (Integrated Delivery System)之組織聯盟，而2005年7月全民健康保險開始實施雙向轉診制度，以建立完善的醫療照護體系，促使健保資源能有效合理運用，其可藉由社區醫療群間的垂直與水平整合，提供體系內的納保人最周全性及連續性的醫療照護。而美國採取管理式醫療被認為是最能控制醫療費用的利器，英國亦已成立基層醫療信託，以充分利用基層醫療，避免跨區就醫。反觀台灣目前現況，醫療層級間仍缺乏互信及合作機制，轉診制度似乎難以落實。故如何提升其有效合作效率，是目前重要議題。
本研究目的為建立社區醫療群合作夥伴選擇模式，以提供社區醫療群於選擇夥伴時的參考。由於社區醫療群選擇合作夥伴之評估準則，為具有多準則與模糊之特性，故研究方法結合網絡分析法(Analytic Network Process)，及模糊理論(Fuzzy Theory)應用，再依相關醫管人員參與評估架構建立，進行最適合作夥伴選擇準則與合作類型選取，並經由評量各項目之相對重要性，進而建構社區醫療群合作夥伴評估模式，實有助於未來社區醫療群選擇與其長久的合作關係建立的預期效果。

In order to fulfill the classified medicine and a referral systern. In 2003, Department of Health implemented “The Experimental Plan for the System of Integrated Care of Family Physicians in National Health Insurance”. And in 2005, Department of Health started to carry out a referral system. Based on the establishment of “Integrated Delivery System” embedded in the local community, the plan aims to promote the vertical and horizontal integration of primary medical care and hospital, advance the quality of medical service, and control the growth of medical expenditure in the whole society. Management based medical system adopted by American government has been considered as the effective weapon for controlling medical cost. The UK has transformed basic medical groups to basic medical trust, which emphasizes family doctors, and encourage doctors to join a group practice so as to widely use basic medical equipment. According to the statistics data, the effect of referral system in Taiwan is not clear and ideal. Therefore, how to constitute a community healthcare group and improve its efficiency becomes cooperation became an important issue .
The main purpose of this study is to set up a model for selecting cooperation partners in Community Healthcare Group. It could offer reference to different Community Healthcare Group, when they select cooperation partners. Selecting the cooperation partners for a Community Health Group involve criteria and the fuzzy concepts in. However, this plan’s research methods combine Analytic Network Process and Fuzzy Theory. By using the fuzzy multiple-criteria decision-making with doctors’ and hospital experts’ opinions, the suggested can to select proper cooperation partners in Community Healthcare Group in the future. Beside, the expected result could construct cooperative relationship for a long time.
