以模糊模型檢測老年人認知能力之研究

摘要


簡短式智能評估是檢測失智症中最常見的量表之一,包含有五大施測項目:定向感、訊息登錄、注意力與計算力、記憶力、語言理解和視覺繪圖能力。本研究先針對簡短式智能評估結果提出探勘高效益項目集的方法,以分析各施測項目中的關聯性。相較於過往認知能力評估的研究,多半以問卷搭配畫圖,鮮少研究利用文字為檢測的方式,本研究於平板電腦中針對視覺繪圖能力設計永字測驗來輔助施測,其目的不僅可以達到認知能力的施測,亦可利用平板電腦來檢測受測者描繪筆劃時手部顫抖程度,根據永字八法來施測,共可歸納成六大面向指標項目,包含:每一筆畫的長度、斜率偏差度、預備書寫的反應時間、每一筆畫所歷程的時間、書寫時的顫抖程度以及書寫時超出書寫範圍的程度,施測結果再依據所提之模糊模型來量化受測者的認知能力。最後使用T檢定來檢定CDR等於0和CDR大於0兩族群的受測者其永字測驗結果各項目的顯著性,並且將達到顯著性的項目結合簡短式智能測驗各項目來探勘高效益項目集,實驗結果顯示所提出之系統可以做為醫療人員看診的輔助參考。

Data mining as the name suggests is the process of digging or mining some information from the vast amount of data. This is also being done on the records of depression till now but it is analyzed that elderly people suffering from depression may deal with many other cognitive impairments. One of the possibilities is it can be an early symptom of dementia. It is considered one of the highest risk factors for the cause of dementia. Mini-Mental State Examination (MMSE) is commonly used in diagnorsis to screen for dementia. It includes test of orientation, action, attention, language, short term memory and drawing. High utility item set mining algorithms is used to discover the common associations from the data gathered by this screening test. This study is aimed on designing a Yong-word test on Tablet PC to assist this test. Past studies have focused on cognitive tests that include drawing or writing, few studies have used Chinese words to evaluate elderly cognition state but the Yong-word test apart for evaluating elderly cognition, can use the Tablet PC to detect the degree of shaking hands when the subject is writing. It measures the amount that a subject has written or length of each stroke in case of Chinese characters, degree of slope deviation, reaction time, the time course of each stroke and level of trembling. The results are evaluated using the proposed fuzzy inference model. The results extracted shows that the proposed system can be used by the doctors or medical staff to diagnose dementia before out patient service and save patients from going into the worsen states of cognitive impairment.
