台灣漢生病患口腔健康狀況與相關生活品質研究

摘要

本研究主要目的是探討樂生療養院長期集中收治漢生病老人，其口腔健康相關生活品質之現況與影響因素。
本研究針對樂生療養院，以立意取樣方式，收集橫斷式資料，個案為願意接受口腔檢查及問卷訪問、意識清楚之漢生病老人。研究資料包括臨床口腔檢查資料及問卷訪查資料。共發放問卷100份，有效問卷88份，有效回收率88.0%。
本研究採SPSS 19.0統計套裝軟體進行資料分析，分析結果顯示，樂生療養院漢生病老人整體口腔健康相關生活品質平均分數為46.92 (±8.01) ，50分以下者佔65.9%；單變相差異分析顯示，在整體口腔健康相關生活品質上，教育程度、失能程度、有無糖尿病、口腔清潔工具的使用等變項有顯著差異；而多變相迴歸分析顯示，在控制人口學因素後，以癌症、進食種類及口腔清潔工具的使用和口腔健康生活品質分數有顯著相關，預測整體口腔健康相關生活品質的總解釋變異量為48.6%。
影響樂生療養院漢生病老人口腔健康相關生活品質的因素主要為口腔清潔工具的使用及進食種類，建議相關政府單位及照護機構盡快與口腔醫療團隊合作，介入相關輔具或潔牙模式，為樂生療養院機構老人提供確實有效的口腔健康福利。

The purpose of this research was to assess the oral-health related quality of life among the elder with leprosy in Lo-Sheng Sanatorium and Hospital and to identify its current status and relevant factors.
Cognitively coherent elder with leprosy willing to accept oral exam and questionnaire surveys in Lo-Sheng Sanatorium and Hospital were recruited into the study using a purposive sampling technique and cross-sectional data was collected. Data included clinical oral examination and questionnaire survey. A total of 100 questionnaires were distributed and 88 were valid, i.e. the effective return rate was 88.0%.
This research data was analyzed by SPSS19.0 statistical package and results showed that the average score of these subjects was 46.92 (±8.01), with 65.9% of subjects scored less than 50. Univariate difference analysis showed that, in the overall oral health-related quality of life, variables such as education attainment, degree of disability, with or without diabetes, the use of tools for oral hygiene were significantly different; multivariate linear regression showed that, after controlling for demographic characteristics, cancers, kinds of eating food, the use of oral hygiene tools and the oral-health related quality of life scores are significantly correlated. The Model explained the total variance.
The use of oral hygiene tools and kinds of eating food are the most important factors that affect the oral-health related quality of life among the elder with leprosy in Lo-Sheng Sanatorium and Hospital. Dental health welfare is in the need of intervening by conducting related aids or dental mode for relevant government agencies and care facilities in cooperation with oral health care teams.
