海峽兩岸臨床醫學生職業安全防護認知現狀及影響因素分析

摘要


醫務人員職業安全是當今醫療領域的重要議題,而醫學生在這方面受到的重視相對較少。醫學生入行前由於經驗不足容易忽視工作背後的潛藏危機,很容易受到集各種有害因素並存的工作環境的影響,需要引起我們的高度重視。本研究透過问卷调查的方式,分別從醫學生基本資料和職業安全認知情況(重要性判斷、認知得分)展開,對福建醫科大學附屬第二醫院和臺北市立萬芳醫院、衛生福利部雙和醫院這兩家台北醫學大學附屬醫院之見實習醫學生進行職業安全認知相关之調查。結果顯示,在重要性認知上大陸醫學生認為“非常重要”比例最高的為“掌握發生感染事件的自我防護途徑”,達73.3%,最低的為“實習前疫苗接種”,僅48.3%;台灣醫學生認為“非常重要”比例最高的為“合理輪班和工作時間”,達65.9%,最低的為“人文通識課程對於自身職業安全的重要性”,僅26.8%。性別、在校期間接受職業安全教育、見實習期間接受職業安全教育這幾個變項認知得分均無顯著差異,僅地區認知得分存在顯著差異,台灣認知得分高於大陸。此外影響職業安全認知得分的主要因素為實習時間,大陸醫學生實習時間為認知得分的負性因素。因此我們根據調查結果發現當今醫學生職業安全防護認知所存在的問題,并從個人防護、醫療院校、社會等方面思考提高醫學生職業安全認知之對策,進而完善當今醫學生職業安全現狀。

Occupational safety of medical staff is an important issue in today’s medical field, and medical students receive less attention in this respect. Due to lack of experience, medical students can easily ignore the hidden crisis behind their work and are easily influenced by the working environment with various harmful factors. Therefore, it needs our attention. Through the questionnaire survey, respectively from the basic situation of medical students and the situation of occupational safety cognitive(importance judgement and cognitive score),this study investigated the occupational safety cognition of medical students from The Second Affiliated Hospital of Fujian Medical University and Taipei Wan Fang Hospital, Ministry of Health and Welfare Shuanghe Hospital. Study results show in the importance cognitive, the highest proportion that students in mainland China considered "Very important" as "Grasping the way of self-protection in case of infection" reaching 73.3%, the lowest for "Vaccination before internship" only 48.3%. The highest proportion that students in Taiwan considered "Very important " as " Equitable shift arrangement and working hours" reaching 65.9%, and the lowest for " The importance of humanities general education curriculum for occupational safety" only 26.8%. There was no significant difference between these variables and cognitives scores in terms of gender, occupation safety education during school, occupational safety education during the internship, only the region and cognitive scores were significantly different and the score of Taiwan was higher than mainland China. In addition, the main factor affecting occupational safety awareness score is the internship time, and the internship time in mainland China is the negative factor of cognitive score. Therefore, according to the results of the investigation, we revealed the problems existing in occupational safety cognitive of medical students today and consider the countermeasures to improve occupational safety of medical students from personal protection, medical institutions and social areas to improve the occupational safety of medical students.
