醫令資訊系統改善了傳統醫師開立處方過程可能產生的問題,但也衍生出電腦資訊系統與使用者間的溝通失誤,其最大的影響在於它可能引起醫療用藥疏失。國內外大部份研究,多以提升醫療服務品質、系統功能、經營績效評估、以及與電子病歷相關的系統技術整合或異質平台整合為主,較少從人機介面設計的觀點上進行研究。本研究主要以人機介面觀點,探討醫令資訊系統引發醫療用藥疏失的因素,藉以尋求改善與預防疏失的方法,作為從事醫令資訊系統使用者介面設計時的參考,進而降低醫療用藥疏失發生的可能。 在研究方法主要運用次級資料分析法,以及使用性工程中的使用性評估方法為執行依據,分為三個階段進行評估,於「釐清階段」運用前測、問卷調查法,收集使用者的看法及問題權重、於「採證階段」運用放聲思考法,取得使用者操作行為的錄影記錄,爾後再於「確認階段」執行專家訪談法,完整執行醫令資訊系統的介面評估研究。研究成果包括:1. 獲得醫令資訊系統人機介面的評估效標。2. 確認醫令資訊系統人機介面與醫療用藥疏失的相關性並取得疏失發生因素。3.提出醫令資訊系統介面設計建議。 4. 提出使用者與使用性專家認知的差異與建議,作為後續研究的參考。
The Physician Order Entry Systems (POES) have replaced the traditional handwritten prescription process and improve the efficiency of hospitals. However, medication errors may arise from poorly designed POES. While most researches of information technology in medication errors are focused on quality of medication services, system functions, evaluation of management performance and integration of electronic medical records, researches discussing preventing medication errors by interface design engineering of POES are rare. This dissertation surveys possible factors triggering medication errors of POES from the viewpoint of human-computer interface design. The following results are obtained: 1. The evaluation categories to evaluate the interface design of POES are deduced; 2. The factors causing medication errors derived from inadequate interface design of POES are analyzed by the data collected from the suggestions of users and usability experts; 3. Interface design principles constructing safer and more reliable POES’es are proposed; and 4. Suggestions of users and usability experts are compared to reveal the difference and conflicts to understand the key points in designing better POES’es.
