摘要

目前護理人力短缺是全球面臨之嚴重議題，於健康照護體系中，使用技術混合照護模式於護理人力中加入照顧服務員，以解決護理人力不足、確保照護品質及減少醫療成本的問題是醫院管理趨勢。本論文探討技術混合照護模式(The Skill Mix Model)提昇護理人力資源運用效率，主要的研究方法為文獻分析法，輔以行政院衛生署桃園醫院呼吸照護病房之個案分析。研究結果顯示：1.人力成本上，因導入技術混合照護模式而配置的照顧服務員人力成本，相較原有應配置護理人員人力成本，顯示每月可節省約150,000元之成本效益；2.醫療品質指標，整體數據低於未參加技術混合模式的病人；3.護理人員對護理照護模式的介入較有信心讓病人得到滿意；4.護理工作可突顯護理全人照護之獨特性及專業感；5.減輕住院病人及家屬之經濟與照顧之身心壓力負荷

A procedure is Nursing shortage is a critical problem worldwide. In the healthcare delivery system, it is a growing trend in hospital-based administration to introduce the Skill Mix Model (SMM) which nurse aids (NAs) were applied in addition to original nursing staff as a strategy for nursing shortage, care quality maintenance and medical cost reduction.The purpose of this study is to investigate the efficiency-enhancement to nursing human resources after using SMM. The methodology employed include literature analysis and case study analysis in the respiratory care center of Tao-Yuan General Hospital which is one of the Department of Health hospitals of Executive Yuan of R.O.C. We conclude the results as follows: The monthly labor cost of the SMM group decreased by150,000 NT dollars approximately in comparison with the non-SMM group.The overall healthcare quality indicators were inferior in the non-SMM group.
The nursing staff became more confident of patient-care satisfaction after using the SMM. The unique and profession of nursing work was more emphasized in holistic care system.The stress loading of patients and family members was alleviated economically, physically and spiritually.
