摘要


為了解決在移動裝置(Palm OS PDA, cellular phone or Pocket PC)上資源較為不足的限制,我們重新設計了一個新一代的臨床藥品動力學的軟體。此軟體不只保留了在JavaPK for Mobile Devices的所有功能,也加入了一個可以讓使用者依據群體藥品動力學的參數自行定義藥品動力學模式的介面。本研究使用Java Platform, Standard Edition (Java SE, v1.4.2)來做為軟體開發的程式語言。在程式中使用了兩個方法來估算個體的藥品動力學參數:Sawchuk-Zaske方法使用了兩個在穩定狀態下抽測的血中濃度來計算aminoglycosides及vancomycin的個體藥品動力學參數;Bayesian方法只需一點血中濃度即可估算藥品動力學參數。在使用者自行定義的模型方面,則使用了Java數學運算式解析器來解析使用者定義的公式。自動錯誤檢查的功能也設計來輔助使用者使用這套軟體。我們將這套軟體稱為JavaPK for Desktop (JPKD)。JPKD已經成功地被開發出來且可以在MS Windows, Mac OS X, Linux OS 及任何Java支援的桌上型電腦平台上執行。除了在JavaPK for Mobile Devices已建置的七個藥物外,於JPKD另外內建八個藥物(indinavir, ritonavir, enfuvirtide, tacrolimus, everolimus, carbamazepine, lithium 與 theophylline)來供作藥物血中濃度監測之用。使用這套軟體,使用者可以根據群體藥品動力學的研究來定義新的藥品動力學模式。這些模型是可修改的且可以利用Bayesian方法來估算藥品動力學參數。在使用已經定義好的模型時,我們提供了一個表格來讓使用者進行批次的計算。JPKD計算的結果可以匯出成.pdf或.csv兩種檔案格式。簡而言之,JavaPK for Desktop在藥品血中濃度監測的服務方面,是一套非常容易使用且功能強大的工具軟體。JPKD已經在2006年2月發表了,它是一套使用者贊助軟體,並可以從http://jpkd.pkpd.org.tw來下載。

To solve the problem of limitations for the resource of mobile devices (Palm OS PDA, cellular phone or Pocket PC), we were motivated to redesign a new generation clinical pharmacokinetic software which not only preserved all functions of previous JavaPK for Mobile Devices, but incorporated with an user’s interface to define their own models using population PK parameters. We used Java Platform, Standard Edition (Java SE, v1.4.2) as our programming language. Individual PK parameters can be estimated by two algorithms: Sawchuk-Zaske method which uses two sampling concentrations at steady-state to calculate individual PK parameters in aminoglycosides and vancomycin; and Bayesian estimation algorithm which provides estimation of PK parameters for only with one sampling concentration. In the user-defined model, we used a Java mathematical expression parser to parse all users’ defined equations. Automatically error checking functions were also built to guide users step by step. We call this software as JavaPK for Desktop (JPKD). JPKD has been successfully created and it can be run on MS Windows, Mac OS X and Linux OS or any other Java supported desktop platforms. In addition to original seven drugs built in mobile edition, we added another eight drugs (indinavir, ritonavir, enfuvirtide, tacrolimus, everolimus, carbamazepine, lithium and theophylline) for TDM in JPKD. With JPKD, users can define a new drug model based on population PK study. The model can be edited and run to estimate PK parameters by Bayesian algorithm. When users execute a defined model, a spreadsheet is provided for batch estimation. The calculated results of JPKD can be saved as .pdf or .csv format. In summary, JavaPK for Desktop is a very easy-to-use and powerful tool for TDM services. JPKD has been released at Feb. 2006, and can be downloaded from its website http://jpkd.pkpd.org.tw as a donateware.
