探討藥師在白班與夜班輪值時自主神經功能的平 衡狀態及四君子湯在調整自主神經功能平衡改善結果

摘要


本研究的主要目的為探討藥師在白班與夜班輪值時其自主神經平衡狀態作為研究主題。另外進一步的探討四君子湯與氣虛者治療之間的相關性。 方法:自主神經直接影響整體機能活動,如唾液分泌,血壓,心率,也間接影響體溫,呼吸等。本研究選用唾液量、心博間隔 (脈率)、體溫、呼吸間隔以及血壓為指標,既反映中醫的虛證,也反應自主神經平衡的關係。本研究以南台灣某區域教學醫院之藥師爲研究對象,其定義及人數分別為:12位非藥師人員為對照組; 21位同時輪值白班與夜班藥師的藥師組。分析組別分為四組,分別為:輪值白班之正常對照組,輪值夜班之正常對照組,輪值白班之藥師組,輪值夜班之藥師組共四組,實驗設計在四君子湯給藥前及給藥後均進行相關測試。每位受試者於簽署受試同意書後,本研究應用Wenger自主神經平衡因子分析法做檢測,結果可得出自主神經功能平衡指數(= y值)利用y值的變化即可檢測出各組自主神經功能平衡指數的改變。 結果: (1)利用Wenger自主神經平衡因子分析法觀察到輪值夜班之藥師組服藥前y值(-1.27±0.19) (**p <0.01)呈現副交感神經興奮狀態。 (2)服用四君子湯四週後輪值夜班之藥師組y值(-0.2±0.17)(**p <0.01),因此四君子湯作為氣虛者的治療結果有明顯的調節作用。 結論:從本研究結果表明,絕大部分輪值夜班之藥師其自主神經平衡指數y值低於正常值,處於副交感神經功能活動增強或亢進的狀態。而四君子湯對於輪值夜班之藥師氣虛症患者自主神經功能狀態具有明顯的調節作用,特別是能調節氣虛症患者的副交感神經功能狀態。

The purpose of this study is a discussion of the function of autonomic system related to balance condition about day and night shift pharmacists what is different between them and general person. Besides, this study is also discussed the relation between Si Jun Zi Tang (SJZT) and treatment the participants of Qi deficient syndrome group. Methods: Autonomic systems directly affected all the physiological function (e.g. secretion of saliva, blood pressure, pulse rate) and indirectly affected function such as temperature and breath, etc. This study took secretion of saliva, heart rate (pulse rate), temperature, respiratory interval, and blood pressure as a target which not only reflected the Chinese medical science of Qi-deficient but also reflected balance condition of autonomic system. Participants were pharmacists and general staff of a pedagogic hospital in of Southern Taiwan Pedagogical Hospital. Analyses divided into four groups which was normal contrast group of day shift, normal contrast group of night shift group in general staff; day shift pharmacists group and night shift pharmacists group in pharmacist. The experiment is designed that before being treated and after being treated for each experimental group. The number of participants was twelve in normal contrast group and twenty one in pharmacists group. When each participant signed agreement, we examined “Wenger’s equilibrium factor autonomic system analysis” in each experimental group that will get equilibrium index, it means y value. Results: (1) Using “Wenger’s equilibrium factor autonomic system analysis” to observe the constitution of group of night shift pharmacists for pre-treatment, y value equal to -1.27±0.19, (**p<0.01) that was part of exciting of parasympathetic. (2) After 4 weeks treatment for SJZT, the y vale equal to -0.2±0.17, (**p <0.01). The results showed it can adjust the participants of night shift pharmacists for Qi- deficient. Conclusion: The results of this study showed the constitution of most night shift pharmacists who the activity of parasympathetic nervous system increased obviously and y value lower than normal value. SJZT can significantly adjust the constitution of a night-shift pharmacist who is Qi-deficient, especially parasympathetic hyper function
