摘要

隨著時代進步，生活品質愈來愈受到重視；在牙科方面不再只是解決痛的問題，對於美觀與咬合功能的重建，更是提昇生活品質的指標。
牙周病所造成的牙齒功能喪失，除了導因於牙菌斑中細菌的侵犯之外，常常伴隨著齒列不整齊、後牙咬合塌陷等問題，這就所謂的二級咬合性傷害。所以常常在牙周基本治療與牙周手術之外，需要配合矯正歪斜的牙齒，或是介入治療性的暫時性義齒，重建咬合高度，把咬合性傷害去除，才能較完整的治療牙周與咬合功能喪失的病人。
本論文提出了十一個局部或是全口重建的病人，其中包括廣泛型慢性牙周病、廣泛型侵犯性牙周病。在治療的過程中，除了牙周補綴治療之外，其中四個病人介入了矯正治療，改善牙齒方向與排列以利將來的預後；有八個病人介入了人工植牙的治療，以增加整體咬合支持。治療計劃的設定除了考量牙齒的牙周狀況、咬合支持關係之外，也需要考量病人的身體狀況與經濟狀況，達到以病人為導向的全口重建治療目標。

With the advanced technology, people have placed greater emphasis on quality of life. In Dentistry, not only do we deal with pain but make more effort to the functional occlusion and esthetics, which are burgeoning indices of quality of life for modern people.
The loss of dental function caused by periodontitis results not only from the invasion of bacteria in the plaque but often from the existence of structural problems, such as malocclusion and posterior bite collapse, which are referred to secondary trauma from occulsion. In addition to basic periodontal treatment and surgery, we also need to employ orthodontic, prosthetic and occlusal therapy to eliminate the occlusal trauma. Then we can do a comprehensive treatment work for these patients with periodontal and occlusal disorders.
Eleven cases with partial or full mouth rehabilitation were presented in this thesis, including generalized chronic periodontitis and generalized aggressive periodontitis. There are four patients involved orthodontic treatment that improves prognosis by rounding and leveling. There are eight patients involved implant therapies that make the occlusal support better. In addition to consider the periodontal status and occlusal support, we must take the financial status and systemic disease of the patients into consider. This is the so-called patient-oriented full mouth rehabilitation treatment goal.
