某醫學中心產前教育對母乳哺餵成效之探討

本研究主要目的檢測產前教育對婦女母乳哺餵之成效，了解婦女有無接受產前教育對純母乳哺餵、混餵、配方奶等三種哺餵方式之影響及婦女成功母乳哺餵的預測因子。採回溯性追蹤式世代研究設計，資料來源為南部某醫學中心2014年1月至2016年12月間有通報紀錄之生產產婦資料。研究結果發現：（1）產前教育能提高產後純母乳哺餵比率。（2）純母乳哺餵與生產方式、試管嬰兒、早產兒、雙胞胎及高血壓有相關。（3）影響成功純母乳哺餵的因子為生產方式，剖腹產婦女與純母乳哺餵為負相關。（4）混餵之哺餵方式與產前教育、試管嬰兒有相關。（5）配方奶哺餵方式與生產方式、試管嬰兒、早產兒及雙胞胎有相關。本研究結果能在產前教育與產科護理中，提供產科護理師了解生產婦女母乳哺餵與各相關變項之相關性，作為照護上之參考依據，並能預知產婦之狀況，給與產婦適切的哺餵個別性照護，以提升母乳哺餵比率，增進嬰兒及婦女健康。

This study explored the effects of prenatal education on breastfeeding among women. The objective was to understand whether receiving prenatal education affects the choice of exclusive breastfeeding, mixed feeding, or formula feeding and to determine the predictors of successful breastfeeding among women. As a retrospective cohort study, this research collected data from medical records of pregnant women registered between January 2014 and December 2016 at a medical center in southern Taiwan. The results reveal that: (1) prenatal education increases the postpartum exclusive breastfeeding rate; (2) exclusive breastfeeding is related to childbirth method, in vitro fertilization (IVF), premature infants, twins, and hypertension; (3) childbirth method is a factor related to successful exclusive breastfeeding, and cesarean section is negatively correlated with breastfeeding; (4) mixed feeding is related to prenatal education and IVF; and, (5) formula feeding is related to childbirth method, IVF, premature infants, and twins. The findings of this study can serve as a reference in the fields of prenatal education and obstetrical nursing, providing obstetrics nurses with an understanding of the correlation between breastfeeding and relevant variables for new mothers. These findings enable obstetrics nurses to predict the conditions of mothers after childbirth and provide appropriate individual care regarding infant feeding. Such care can elevate the breastfeeding rate and further improve the health of infants and women.
