高齡孕婦對羊膜穿刺術的知識及選擇意圖之探討

高齡孕婦過去的產前染色體檢測項目以侵入性為主，羊膜穿刺及絨毛膜穿刺是胎兒染色體檢測的方法，以羊膜穿刺為常見，但有破水、流產及感染的風險。本研究旨在瞭解高齡孕婦對羊膜穿刺術知識程度及選擇意圖。本研究採橫斷性描述性研究，採自編的結構式問卷對高齡孕婦作問卷調查，以個別填答問卷方式進行資料收集，進行高齡孕婦對羊膜穿刺術的知識及選擇意圖之相關性探討。我們以台南地區的高齡孕婦為研究對象，收案地點在台南市一婦幼專科醫院，調查時間自107年05月13日起至12月31日，收案對象資格為懷孕滿10週時年齡達34歲者(足歲)，實際收案人數為140位。問卷包含有羊膜穿刺術知識問卷9題、羊膜穿刺術選擇意圖問卷5題與高齡孕婦的基本資料14題，所收集的資料以SPSS for Windows Release 18.0軟體進行資料處理分析，以敘述性分析、單因子變異數分析、皮爾遜積差相關分析等作為統計方法。研究結果顯示羊膜穿刺知識得分為6.55±1.29/滿分為9分、羊膜穿刺選擇意圖得分為16.22±4.06/滿分為20分，有無做過羊膜穿刺與羊膜穿刺選擇意圖總分呈中度正相關(r=.401**)，高齡孕婦對羊膜穿刺術的選擇意圖影響因素為過去有無做過羊膜穿刺術。本研究結果可提供臨床醫護人員瞭解高齡孕婦對羊膜穿刺術知識的瞭解情況及其選擇意圖之影響因素。

Prenatal chromosomal detection programs for the advanced maternal age women were mainly invasive in the past. An amniocentesis and a chorionic puncture were the methods of fetal chromosome detection. An amniocentesis method is common, but there was a risk of water, abortion and infection. This study was to explore the knowledge and intention of amniocentesis among the advanced maternal age women. This study is a cross-sectional convenience survey to utilize a self-administered structured questionnaire for the advanced maternal age women to obtain data about the knowledge and intention of the prenatal amniocentesis testing. One hundred and fifty advanced maternal age women from a women and children hospital in Tainan were recruited. Descriptive statistics, including frequency, percentage, mean value, standard deviation, analytical statistics, ANOVA, Pearson’s product-moment correlation analysis will be performed by the SPSS for Windows Release 18.0 software package. The results of this study showed that the average score of the knowledge of Amniocentesis was 6.55 ± 1.29 (full grade: 9). The average score of intention of amniocentesis was 16.22 ± 4.06 (full grade: 20). The influencing factor of the knowledge of Amniocentesis was the past experience of amniocentesis. The results of this study can provide the information to understand the reference of the knowledge and intention of amniocentesis among the advanced maternal age women in the women and children hospitals.
