軍人穿不同陸軍用鞋行走時的足底壓力特徵與特製鞋內墊的效應

摘要


軍事訓練中,因過度使用下肢而造傷害是最常見的問題。大部份的初期軍事訓練會快速增加跑步的量及強度,進而造成過度使用下肢而導致傷害,而根據過去研究發現,護具的使用對於過度使用下肢造成的傷害具有正面保護效果。因此,許多學者建議應研發適當的防護策略以降低其傷害的發生。本研究將根據國軍基本戰技訓練時的需求與考量,比較軍人穿著特製鞋墊對於足底壓力參數的影響。方法:十五位軍官藉由底足底壓力測量系統擷取足底最大對地作用力、最大足底壓力、力量時間積分值、最大平均壓力。結果:本研究結果,發現布鞋的最大平均壓力值有明顯高於軍靴(p<.05)。此外,在鞋墊的介入下發現,最大足底壓力和最大平均壓力值皆有明顯的高於原無鞋墊介入值(p<.05)。而力量時間積分值有明顯的低於原無鞋墊介入值(p <.05)。結論:根據本實驗結果,建議使用特製鞋墊或護具來降低傷害的產生,但仍需要長期的追蹤來得到更完善的證據。

Background: Lower extremity injuries which caused by the excessive use are the most common cause of problems in military training. Military training mode usually increase in training intensity rapidly in short time interval. It was the reason of lower extremity injuries. According to past studies, the use of orthotic for lower extremity injuries had a positive protective effect. Therefore, many researchers suggest that the appropriate protection can decrease the injuries happen. Purpose: This study aimed to compare the effect of custom made insoles on plantar pressure parameters for the soldiers. Methods: The quantify of the maximum force, peak pressure, force-time integrals, maximum mean pressure and mean force for different specific foot masks in the fifteen military officer trainees were assessed by Commercial in-sole plantar pressure measurement system (Pedar-x, Novel gmbh, Munich Germany). Result: We found that the maximum force, peak pressure, force-time integrals, maximum mean pressure and mean force were increase with load carriage significantly (p<0.05). The maximum mean pressure in shoes was significant higher than boots (p<0.05) and the force-time integrals of orthotic was decrease significantly (p<0.05). Conclusion: According to our results, recommend using custom made insoles or brace to reduce the injury. It was need more evidence in future.
