植牙患者對於人工植牙醫療之認知及態度研究:以高雄市地區為例

摘要


研究目的:了解植牙患者對於植牙的認知、態度及其影響因素。 材料與方法:本研究屬橫斷面研究,以高醫附設醫院及牙科診所為抽樣對象,共收集166位植牙患者(男性78人,女性88人,平均年齡52.1±12.35歲)進行問卷調查,執行期間為民國102年8月至9月份。問卷內容包含患者人口特質、口腔狀況及照護行為、植牙經驗及資訊來源、植牙認知及態度,使用STATA 11.2軟體進行統計分析。 結果:植牙患者男女比1:1.13,以45至64歲之中年患者居多(61.4%),下顎後牙比例最高(37.0%)。多變項迴歸分析發現,口腔健康狀況良好、有定期回診以及對植牙美觀滿意者,植牙認知較高(p<0.05);他人植牙經驗表示非常滿意者,患者植牙態度較正向(p<0.05)。研究中亦發現患者對於植牙禁忌症的認知不足,主要資訊來源為牙醫師(81.9%)及媒體(42.8%)。植牙認知及植牙態度之間並無顯著的線性關係及相關性(r=0.0857, p=0.272)。 結論:認知部分,建議牙醫師提供正確植牙資訊給患者及媒體、加強植牙禁忌症衛教、提升患者口腔健康狀況及定期回診習慣。在植牙評估時需詳細說明手術過程及風險、其他治療選項、術後可能之不適及併發症,提供患者作為選擇植牙前之判斷依據。態度部分,建議強化醫病溝通,提升醫療專業形象及技術,以傳播良好口碑及評價,提升患者植牙意願,進而推薦他人植牙,使植牙更普遍為大眾所接受。

Objective:The aim of this survey was to assess the awareness and attitude toward implant therapy of the implant patients and their influencing factors. Materials and methods:This cross-sectional study recruited 166 subjects(78 males, 88 females, mean age: 52.1 ± 12.35 years) treated with oral implants in Kaohsiung Medical University Hospital and three dental clinics during Aug. to Sep., 2013. The questionnaire included the demography, oral health status, oral care behavior, implant experiences, information source, awareness and attitude of the patients toward dental implants. The STATA 11.2 software was used for statistical analysis. Results:Most patients were aged 45 to 64, and the implant sites were mainly at lower posterior area(37%). The factors influencing patients’ awareness were: oral health status, regular recall visits and aesthetic satisfaction(p <0.05). The positive attitude toward implants was found, and the influencing factor was the satisfaction of their friends and relatives (p <0.05).The patients had a lack of knowledge about the contraindications for implant therapy. The main information sources of implant therapy were dentists and media. There’s no correlation and linear relationship between the awareness and attitude (r=0.0857, p = 0.272). Conclusion:Our study recommended to provide correct information of implant therapy to patients and media, and to improve patients’ dental health and keep regular follow-up after implant placement. Moreover, detailed explaination is necessary before implant surgery, including the contraindications, risks, treatment options, post-operative discomfort and complications, so that patients could make decisions on their own judgements. Dentists should increase the communication with patients, and the professional technique as well.Thus, the willingness of patients to receive implant therapy would be increased.
