嚼食檳榔者口腔保健知識、態度、行為 及口腔健康生活品質相關性之探討:以屏東縣萬巒鄉為例

背景: 在台灣嚼食檳榔習慣已成為嚼食檳榔者社交及日常生活的一部 分,國內外研究顯示嚼食檳榔者的牙周指數顯著高於非嚼食者,口 腔衛生比無嚼食檳榔者差。 目的: 本研究將探討嚼食檳榔者口腔保健知識、態度、行為及口腔健 康生活品質的相關性。 方法: 本研究為橫斷性研究設計,以滾雪球方法進行收案,最後共有 111 位嚼食檳榔者及 194 位無嚼食檳榔者參加。使用自填式結構式 問卷收集基本人口學資料、口腔保健知識、態度、行為及口腔健康 生活品質。採用獨立 t 檢定、卡方檢定分析嚼食檳榔者及無嚼食檳 榔者在口腔保健知識、態度、行為及口腔健康生活品質之差異,並 使用迴歸分析嚼食檳榔者與口腔保健知識、態度、行為及口腔健康 生活品質的相關性。 結果: 迴歸分析調整人口學變項後結果顯示,嚼食檳榔行為(adjβ=-3.4 3; 95%CI=-5.17,-1.69; p<.001)與口腔保健態度有相關,並達統計學 上顯著差異。嚼食檳榔者與使用硬刷毛牙刷(aOR=2.47;95%CI=1.05, 5.81; p=0.038)、無使用含氟牙膏(aOR=2.48; 95%CI=1.04,5.88; p=0. 039)、比較不會有牙科拜訪(aOR=2.16; 95%CI=1.04,4.46; p=0.037)有 顯著相關。 結論: 嚼食檳榔者的口腔保健知識、態度、行為較無嚼食者差。建議 口腔衛教時可針對檳榔嚼食對口腔健康的危害及潔牙工具的使用方 法及運用多加宣導,以提升民眾的口腔健康。

Background: In Taiwan, the habit of betel nut chewing has become a part of social and daily life for betel nut chewers. Domestic and foreign studies show that the periodontal index of betel nut chewers is significantly higher than that of non-chewers, thus their poorer oral hygiene compared to non-chewers. Objectives: This study explored the relevance between the habit of betel nut chewing, oral health knowledge, attitudes, behaviors and oral health related quality of life. Methods: This study is a cross-sectional study and the snowball method was employed for case acceptance. A total of 111 betel nut chewers and 194 non-betel nut chewers participated. The self-filled structured questionnaire was used to collect basic demographic data, oral health knowledge, attitude, behaviors and oral health related quality of life. Independent t-test and chi-square analysis were used to analyze differences between betel nut chewers and non-betel nut chewers in terms of their oral health knowledge, attitude, behaviors and oral health related quality of life. The regression analysis was then applied to analyze the relevance of betel nut chewers’ oral health knowledge, attitude, behaviors and oral health related quality of life. Results: The linear regression analysis results show that betel nut chewing behavior (adjβ=-3.43; 95%CI=-5.17,-1.69; p<.001) is related to oral health attitude, reaching statistically significant differences. Betel nut chewers and the use of toothbrushes with hard bristles (aOR = 2.47;95%CI = 1.05,5.81; p = 0.038), without the use of fluoride toothpaste (aOR = 2.48; 95%CI = 1.04,5.88; p = 0.039) and fewer dental visits (aOR = 2.16;95%CI = 1.04,4.46; p = 0.037) produced significant relevance. Conclusions: The oral health knowledge, attitude and behaviors of betel nut chewers are poorer compared to non-betel nut chewers. It is recommended that propagandas on the harm of betel nut chewing on oral health and tooth cleaning device instructions and usage be strengthened during oral health education in order to improve the or al health of the general public
