組織於製程安全評估中所使用風險矩陣是否有效地揭露可能的最大損失是本研究探討的主要議題。本研究收集國內石化工業區廠商使用的製程安全評估風險矩陣,並對風險矩陣的有效性進行問卷調查。本研究發現各公司現行使用的風險矩陣並未於使用前進行評估。大部份均沿用製程安全評估相關訓練課程講義內容,未就公司性質及規模進行適用性評估檢討。且目前使用的風險矩陣評估面向及可能性/嚴重度未訂定明確的評估程序與制度,以致於影響到嚴重等級判定及改善措施運用。 本研究最後提出對製程安全風險評估的改善建議,俾使一般業者得以據此,再對評估方法進行修正,並明確認定風險的等級,避免造成模糊的定義空間,可更有效的進行風險矩陣的使用,確實評估出高風險項目並加以改善。
This study is aimed to investigate if the risk matrix is well designed and capable to reveal the worst scenario in the risk assessment process in the petrochemical industry. This research collected the risk matrix of process safety assessment used by plants located at petrochemical industrial park and conducted questionnaire survey on the effectiveness of risk matrix. The survey showed that there was no systematic approach when determining risk matrix before risk assessment proceeding in most plants. Most of them just adopted the sample case from the training materials instead of taking operation characteristics into consideration. Without carefully characterizing the rating range of frequency and consequence, assessment results are very likely biased and mislead the recommended corrective actions. This study concluded some recommendations for risk assessment on process safety. Hopefully, they might be adopted to identify the risk rating definitely, be free of unclear definition as well as make the assessment close to reality.
