研究目的:探討台灣運動神經元疾病之流行病學與醫療支出。 方法:從台灣的健保資料庫中,篩選出同時有運動神經元疾病診斷碼與重大傷病身分的就醫資料。所篩選的資料範圍從2000年1月到2005年12月。求得各年齡層與男女性別的個案數,再由新發生的個案數除以內政部所公告的各年齡層與性別人口數,即可得知各年齡層與性別的發生率。由健保資料庫中的數據也可得知醫療支出。 結果:在2000-2005這六年當中,共有1554位運動神經元疾病患者。平均發生率為每年每十萬人口1.05人(從0.95到1.17)。男比女的比率為1.51。男性的平均發生率為每年每十萬人口1.25人,女性為每年每十萬人口0.84人。此發生率會隨著年齡增加而增加,男性到65-69歲這個年齡層達到最高點,女性的發生率則在70-74歲這個年齡層達到最高點,在更老的年齡層發生率則會下降。運動神經元疾病患者的就醫次數與醫療支出,逐年遞增,但住院部分在2005年突然下降。 結論:在2001-2005年當中,台灣的運動神經元疾病平均發生率為每年每十萬人口1.05人。此數據低於白種人的數據。男性的發生率略高於女性,且發生率隨著年齡而增加。隨著台灣人口結構逐漸老化,運動神經元疾病的病患數目與醫療支出在未來數十年當中,預期也會增加。
Objective: To investigate the epidemiology and associated medical expenses of motor neuron diseases in Taiwan. Methods: Data from the National Health Insurance Research Database involving diagnoses of motor neuron diseases and Certificates of Serious Accidents and Diseases were retrieved for January 2000 to December 2005. Age and sex specific incidences of motor neuron diseases were estimated by dividing the incidence number by population data obtained from the Department of Statistics, Ministry of the Interior. Medical expenses associated with the disease and incidence trends were also described during this period. Results: During this six-year period, 1554 patients with motor neuron diseases were identified. The average yearly incidence was 1.05 per 100,000 (range 0.95-1.17). The male to female ratio was 1.51. The incidence rate, which was 1.25 in men and 0.84 in women, tended to increase with age and peaked at the 65-69 range in men and the 70-74 range in women; it decreased thereafter. The counts and expenses of the medical services associated with motor neuron diseases increased year by year, except for a sudden drop in inpatient expenses in 2005. Conclusions: The incidence of motor neuron diseases in Taiwan during 2001-2005 was 1.05 per 100,000/year, which was lower than that of Caucasians. Incidence had moderate male predominance and increased with age. With Taiwan’s aging population, case numbers and medical expenses associated with these diseases are expected to rise in future decades.
