遠距心臟照護(Telecardiology)是指慢性心臟病患於住家中自我量測心電圖上傳至醫療中心由專任醫生控管,當病徵發生時再通知病患回診檢查。本研究將利用雲端運算(Cloud Computing)網路服務無所不在的特性將其應用於遠距心臟照護,提供雲端醫療服務。以OpenMRS電子病歷系統進行心電圖報告系統擴充,提供專任醫生線上心電圖判讀與報告撰寫服務;病患能隨時查閱個人醫療資訊,醫療資訊不用侷限於定點的醫院中。 系統架構分為三個部分,居家端、OpenMRS管理雲與醫院端。居家端病患使用建置於Android平台上的心電圖量測裝置,心電圖以DICOM格式儲存上傳至OpenMRS管理雲。在中華電信hicloud CaaS中建置OpenMRS管理雲,提供個人空間儲存病患心電圖資料、DICOM ECG Viewer顯示DICOM ECG資料、醫生線上判讀與報告撰寫、病患線上查詢健康資訊與E-mail通知服務。醫院端專任醫生經由OpenMRS管理雲通知,登入OpenMRS管理雲,對病患資料進行線上判讀與報告撰寫。 本研究已成功建立OpenMRS管理雲,病患與專任醫生皆可由網際網路連線到OpenMRS管理雲上查看資訊。另外以WebBench與Blitz兩套軟體對OpenMRS管理雲進行多人連線的壓力測試,在多人同時連線情形下系統仍可穩定運作。 關鍵詞:雲端醫療、遠距心臟照護、OpenMRS
Telecardiology means a different way for cardiologist taking care of patients. Via telecardiology, physicians can remotely monitor patients’ cardiac status by reviewing the uploaded ECG records and, if necessary, arrange further medical treatments. This research provides a new service of Telecardiology service with the cloud computing technology. The Cloud Telecardiology System is an extension of open source OpenMRS medical record system. With the ubiquitous features of cloud infrastructure, cardiologist can remotely access and interpret the patients’ ECG records, document the report anytime and anywhere. In this fashion, patients can review the latest health status report timely. With this configuration, the medical care can be decentralized from traditional medical service sites. The Cloud Telecardiology System has three major components: Patient site service, OpenMRS Cloud management, and Caregiver site service. On the patient site, ECG records are collected by an Android tablet system, packed into DICOM format, and then sent to the OpenMRS Cloud system. OpenMRS Cloud provides ECG storage and record management service on Hi-cloud CaaS. An accompanied DICOM ECG viewer helps physician review the ECG records and record the interpretation on the online charting system. Once the ECG interpretation is completed, an email reminder will be issued and patient can access the report from the internet. In this research, the OpenMRS Cloud Telecardiology System has been implemented and tested thoroughly. Stress test also has been conducted by WebBench tool and Blitz web application. The performance shows the system can work robustly upto 312 users operating at the same time.
