眼部斷層掃描(OCT)影像處理於早期青光眼診斷應用

摘要


近年來,眼科疾病中青光眼的患者逐年增加,是臨床上常見的眼睛疾病,同時也是導致失明的主因之一。青光眼疾病是不可逆的,也無法根治,一旦發現此疾病只能在確診時使用藥物控制,減緩病情的惡化。青光眼的造成,是因視神經纖維缺損導致,逐漸引發視野缺損,影響視覺品質,嚴重則將會導致失明。而青光眼在初期時也不易於被發現。正因如此,初期的篩檢診斷出早期青光眼病症,有利於病患提早發現,提早治療。 本研究透過光學同調斷層掃描設備所得之視網膜OCT切片圖,來做一影像處理系統,將切片圖片堆疊成3D立體圖像,讓系統計算出厚度並畫成等高圖,推估驗證此方法是可判斷出缺損位置。結果顯示,驗證方法以及實際運用在臨床數據上可利用假定為厚度下降的10%位置顯示出來並加以標示,並可透過系統表現出視神經纖維缺損的差異,可明確顯示出位置區塊,進而在臨床上判斷時能夠明確的,快速的診斷。

In recent years, the number of patients with glaucoma in eye diseases has increased year by year. It is a common clinical eye disease and one of the main causes of blindness. Glaucoma disease is irreversible and cannot be cured. Once discovered, the disease can only be controlled with drugs at the time of diagnosis to slow down the deterioration of the disease. Glaucoma is caused by defects in optic nerve fibers, which gradually leads to visual field defects, which affects visual quality, and severely leads to blindness. And glaucoma is not easy to be found in the early stage. Because of this, the initial screening to diagnose early glaucoma symptoms is conducive to early detection and early treatment of patients. In this study, an image processing system was used to build an image processing system using retinal OCT slice images obtained by optical coherent tomography equipment. The slice images were stacked into a 3D stereo image, and the system calculated the thickness and plotted it into a contour map. It was estimated and verified that this method was The location of the defect can be judged. The results show that the verification method and actual application in clinical data can be displayed and labeled using the assumed 10% thickness decrease position, and the difference in optic nerve fiber defect can be shown through the system, and the position area can be clearly displayed, and then A clear and rapid diagnosis in clinical judgment.
