護理緣起、初衷不滅-資深護理人員之職涯承諾

Never Forget the Origin and the Initial Goal in the Nursing Profession: The career Commitment of Senior Nursing Staffs

摘要


護理人力不足、護病比不合理、薪資福利與付出不成比例等,導致護理工作量增加及壓力負荷過重,嚴重影響護理人員職業生涯發展。而在面對護理工作環境無法近期改善情況下,仍有一群資深護理人員默默的為護理工作盡心盡力,甚至於將護理工作視為志業,故本研究以質性研究之敘事探究方式收集四位資深護理人員之職業生涯歷程及以護理為志業之過程經驗,資料收集以深度訪談為主、事後電話及個人電子信箱提問做資料補充。逐字稿資料分析採用「整體-內容」分析方法,研究者整理出資深護理人員職業生涯可分為三個階段:護理緣起、職涯歷程之蛻變及初衷不滅,每個階段發展隨研究參與者經歷的不同而變化,對護理職業生涯承諾感受也隨情境、時空改變,由最初的護理緣起:受到父母親的期望/傳統社會文化對女性之刻板印象而被動式的選擇,也可以是現實面考量其工作穩定/專業技能學習、或甚至於外在美好形象吸引產生了對護理工作的憧憬;在投入護理工作後護理工作歷程之蛻變:以轉換心境、勇於承擔、無私奉獻的正向思考力量、支持系統之建立、自我成長與心理調適以跨越工作困境;不論醫療環境或健康政策不斷變動,護理仍以助人及「愛與關懷」的護理本質,來體現護理專業及價值核心的初衷不滅,這三個階段正是資深護理人員於職業生涯歷程中讓自己的生命與生活產生了互為主體的影響。 關鍵字:資深護理人員、職涯承諾、職涯經驗

Abstract: In sufficient nurses, unbalanced ratio of nurses to patients, disproportion of salary to workload and other factors have led to increase in works and heavy burden and stress, which seriously affect the career development of nursing staff. However, despite of unimproved working environment for nurses, some senior nursing staff still do their best in the work and regard it with career devotion. This study was a qualitative research with narratives of four devoted senior nursing staff’s career experience, collected via in-depth interviews, post-facto phone calls and individual email inquiry for supplementary information. The analysis of verbatim record would check the “whole content”. As compiled by researchers, the career life of the senior nursing staff could be divided into three phases: the origin of choice in nursing career, the experience of change in the course of career and the unrelenting career commitment, that each subject might vary due to different experiences and changes, as their commitment in nursing career might change along the situation, time and space. In terms of the origin of selecting a nursing career, many might passively select this career path as the choice of parents’ expectation and traditional/cultural stereotyping of women in society, as a consideration of reality for the job’s stability and professional skill learning, or as simply the envision of good external appearance of being a nurse. But, down the career path after choosing to be a nurse, some faced change in the state of mind to require themselves with courage to bear responsibility and positive thought of selfless dedication in works, that some would need some sort of support system and psychological adjustment in the process of self-maturation, in order to overcome some difficulties in works. Regardless of continuous chaotic changes in medical environment or health policy, nursing is still about helping others with “love and care” to reflect the professionalism and the core value of a nursing career without failing the original intent of such dedication. These are the very three stages the senior nursing staff has gone through in career that very much have significantly impacted on their view of life and daily activities.
