應用德菲層級法於醫療機構環境清潔外包廠商合作經營評選模式

Using DHP to Develop a Selection Model for Outsourcing Suppliers of Medical Environment Clean

摘要


醫療機構納入勞基法加重營運成本的支出,總額預算支付制度實施的醫療給付環境變遷等因素下,採行委外經營的方案,環境清潔於實際運作存在相當落差衍生諸多問題。本研究針對外包(outsourcing)與醫療機構合作經營時,於夥伴關係間互補資源選擇優先考量的重要因素深入探討。 本研究採用德菲層級分析法(Delphic Hierarchy Process;DHP)分兩階段進行。第一階段以文獻探討蒐集決策關鍵因素制訂前測問卷,再經由專家小組進行兩回合的問卷調查,重複施測及回饋控制的模式,篩選出探討醫療機構環境清潔外包廠商合作經營評選因素,以「財務與成本」、「合作與關係」、「專業與營運」、「商譽與法規」等四項準則與十三項指標,規劃設計為評選模式的整體架構;第二階段運用層級分析法(Analytic Hierarchy Process;AHP)進行成對比較矩陣,以Expert Choice 2000決策支援應用軟體,求解層級權重分配與排序。 研究結果歸納出「評選醫療機構環境清潔外包廠商」,第二階層整體權重依序為,財務與成本(權重為0.303)、合作與關係(權重為0.255)、專業與營運(權重為0.234)、商譽與法規(權重為0.207);第三階層13項指標前五項分別為「外包商財務穩定度與風險」、「價格完整及合理性的成本效益分析」、「熟知法條並符合法令規定」、「外包商關係管理的配合能力與配合度」、「履約中的財務管理計畫建立與實施」。以上研究結論作為選擇環境清潔外包廠商優先順位之參酌。

Due to the increased operating costs which caused by the inclusion of medical institutions in the Labor Standards Act and the change of medical payments situation because of the implementation of total budget payment system, business outsourcing become solution. In the environmental clean scope, however, a lot of problems appeared since there is a considerable gap during actual operation. This research focus on the investigation of major consideration factors which associate to the complementary resource of partnership between outsourcing and medical institutions. This research used AHP Delphi (Delphic Hierarchy Process; DHP) and has two stages. The first stage was to formulate a pre-test questionnaire by collecting and exploring the crucial decision factor literature. Then two rounds of questionnaires carried out by a group of experts. Under the repetitive measurement and feedback control condition, screen out the selection factors of outsourcing cleaning companies which would be partnership managing with medical institutions. There are four criterions and thirteen indicators, such as "Financial and costs", "cooperation and relationship", "professional and operational", "goodwill and regulations", which were designed to be the overall configuration of the selection mode. Analytic Hierarchy Process, AHP, was used in second stage to process pair-wise comparison matrix. Furthermore, decision support software applications, Expert Choice 2000, was used to solve level weight distribution and sorting. The results were summarized in the "selection of environmental cleaning outsourcer in medical institutions" which the overall weight of second class is financial and cost (weight of 0.303), cooperation and relationship (weight of 0.255), professional and operational (weight is 0.234). goodwill and regulations (weight 0.207). Top five of the thirteen indicators were " financial stability of outsourcer and risk", " cost-benefit analysis of price integrity and reasonableness ", " familiar and compliance with laws and regulations "," outsourcer relationship management "," performance in the establishment and implementation of financial management plan". The conclusions above is a deliberate priority of environmental cleaning outsourcers selection.
