手術室護理師工作壓力、工作生活品質與工作滿意度之相關性探討

摘要


背景:手術室護理人員是醫療護理產業中複雜性較高的一個專業部門,護理人員的養成不易,需要面對侵入性手術治療,協調應付各種突發情況的發生,因此,手術室護理人員的工作壓力、工作生活品質與工作滿意度都會影響照護品質,故檢視手術室護理工作壓力、工作生活品質與工作滿意度的相關因素是重要的。 目的:探討手術室護理人員工作壓力、工作生活品質與工作滿意度之相關性。 方法:採橫斷式相關性之研究設計,以台灣南部3家醫院手術室護理人員為對象;共100位參加。研究工具包含「工作壓力量表」、「工作生活品質量表」、「護理人員工作滿意度量表」與基本屬性等四份問卷。以SPSS2.0統計軟體進行描述性統計、獨立樣本t檢定、單因子變異數、皮爾森積差相關、複迴歸之統計分析。 結果:共完成100份有效問卷,平均年齡為30.31歲;手術室護理人員的工作壓力感受情形的平均得分為2.64分,工作壓力困擾程度的平均得分為2.40分,工作生活品質滿意度的平均得分為3.29分,工作滿意度的平均得分為3.19分;因年齡、手術室護理工作年資、居住狀況、手術室的房間數量、分科與否、平均每周加班時數、輪班狀況對工作壓力而有所差異;因手術室的房間數量、平均每周加班時數、對工作生活品質而有所差異;因手術室護理工作年資、居住狀況、手術室的房間數量、平均每周加班時數、輪班狀況、對工作滿意度而有所差異。工作壓力感受情形與工作壓力困擾程度呈正相關,工作壓力感受情形及工作壓力困擾程度與工作生活品質呈負相關,工作壓力感受情形與及工作壓力困擾程度工作滿意度呈負相關,工作生活品質與工作滿意度呈正相關。影響手術室護理人員工作壓力之預測因子,包含主要跟刀科別(分科vs不分科)、平均每周加班時數(<4小時vs>4小時);影響手術室護理人員工作生活品質之預測因子,為平均每周加班時數(<4小時vs>4小時);工作滿意度與個人基本屬性與工作特質沒有顯著關係。 臨床應用:本研究結果將提增進了解現行手術室護理人員工作壓力、提高工作滿意度及生活品質進而提升手術室護理品質。

Background: Operating room nursing is a highly complex professional in the medical care industry. The development of the profession of operating room nurses required certain competency, skills, and experience to deal with invasive, operative procedures, as well as unexpected accidents occurred. The work stress, quality of work life, and job satisfaction of operating room nurses are associated with quality of care. Therefore, it is important to identify relevant factors associated with work stress, quality of work life, and job satisfaction. Objectives: The study objective was to examine the association among work stress, quality of work life, and job satisfaction in operating room of nurses. Method: A cross-sectional correlation research design was employed. Operating room nurses in three hospitals in southern Taiwan were recruited, and a total of 100 participants were responded to participant in this study. Five questionnaires were used: the basic demographic and work information, the Work Stress Scale, the Quality of Work Life Scale, and the Nursing Staff Job Satisfaction Scale. Statistical methods were descriptive analyses, bivariate analyses, and multiple regression using SPSS2.0 statistical software. Result: The average age was 30.31 years, the average score on a single item for work stress perceived frequency was 2.64 points, 2.40 points for work stress perceived severity, 3.29 points for quality of work life, and 3.19 point for job satisfaction is. Scores of work stress were significantly different in several demographic variables, including age, years in operating room work, living status, the size of the number of operating rooms, fixed surgical operative room (vs. not), average overtime hours weekly, and shift status. Scores of quality of work life were significantly different in several demographic variables, including the size of the number of operating rooms, and average overtime hours weekly. Scores of job satisfaction were significantly different in several demographic variables, including years in operating room work, living conditions, the size of the number of operating rooms, average overtime hours weekly, and shift status. Work stress perceived frequency was positively correlated with work stress perceived severity; work stress perceived frequency and perceived severity were negatively correlated with quality of work life. Quality of work life was positively correlated with job satisfaction. There were several variables associated with work stress in operating room nurses, which were fixed surgical operative room (vs. not) and overtime hours weekly (<4 hours vs> 4 hours). Only one variables associated with quality of work life was overtime hours weekly (<4 hours vs> 4 hours). There was no significant variable associated with job satisfaction. Clinical implication: The results of this study would be useful in better understanding work stress, quality of work life, and job satisfaction in operating room nurses.
