探討親子同室母乳哺餵之經驗

Exploring Women’s Experiences with Breastfeeding for Parent-child Rooming-in

摘要


背景:於2001年開始,台灣地區進行母嬰親善醫院認證作業,積極提昇國內母乳哺餵率。然而產婦缺乏在生活實境中的母乳哺育學習環境,因此在遇到哺乳問題或角色壓力時,如果無法獲得適時且正確的指導及支持,往往容易停止餵母乳。已有許多研究探討如何提升母乳哺育率與影響母乳哺育的因素,然而卻甚少研究對於產後親子同室哺餵母乳婦女的感受與經驗。 研究目的:瞭解親子同室母親母乳哺餵過程中的真實經歷。 研究方法:本研究以質性研究為基礎,採用Creswell and Poth(2018)資料分析步驟。自2019 年4月至2020年6月,立意取樣至資料飽和為止,共計8 位入住產後護理之家執行親子同室並採取母乳哺餵之產婦參與本研究,藉由半結構式深度訪談收集資料,訪談過程中,使用錄音方式進行資料收集,同時進行編碼與資料分析。 研究結果:親子同室母乳哺餵之經驗中,共有三大主題十項次主題。主題一、沉浸母嬰親善的時刻:(一)母性潛能展現(二)施行的契機 (三)無可取代的小確幸。主題二、母嬰親善的衝擊:(一)生理層面衝擊(二)心理層面衝擊(三)社會層面衝擊。主題三、使命必達的母愛:(一)調整心態(二)調整生活作息(三)尋求社會支持網絡(四)建構自我相關能力。 結論:本研究促使了解親子同室母乳哺餵產婦之內心真實感受,以提供醫療專業人員及政府機關在臨床實務、教育、學術研究及政策上之參考,提供日後有關母乳哺餵的婦女,個別性及有效的護理措施。

Background: Since 2001, "Baby-Friendly Hospital Accreditation" has been implemented in Taiwan in order to increase the rate of breastfeeding. Nevertheless, maternal mothers lack a good learning environment of breastfeeding in daily life. For this reason, when encountering breastfeeding problems or suffering from role stress, they cannot receive correct and timely instructions and support, which might make new mothers stop breastfeeding. Currently, there are many studies trying to know how to increase the rate of breastfeeding and to understand the factors in deciding if mothers want to breastfeed or not. However, there are few studies focusing on women’s experience and feelings about postnatal rooming-in breastfeeding. Purpose: To understand the actual experience and process of room-in breastfeeding for mothers. Method: This study is based on qualitative research and the data analysis of Creswell & Poth (2018) is adopted. From April 2019 to June 2020, purposive sampling was used until the data saturation. In total, 8 women living in postnatal care house and implementing room-in breastfeeding participated in our study. Data were collected by semi-structured in-depth interview. During the interview, the data were saved by tape-recording and processed with coding and data analysis. Results: There are three main themes and ten categories that can be found in the room-in breastfeeding experience: The first main theme is "enjoying the baby-friendly environment":(1)presenting the maternal potential; (2)moment of implementation; (3)irreplaceable happiness.The second main theme is "impact brought by baby-friendly environment": (1) physical impact; (2)psychological impact; (3) social impact.The third main theme is "completing mothers’ missions":(1)changing mindset; (2) adjusting daily routines; (3)asking for social support; (4)self-construction Conclusion: This study is provided to understand the actual feelings of women’s experience of room-in breastfeeding. The information is contributed to medical professionals and government agencies for clinical practice, education, academic research and political purposes in order to have more effective nursing measures for each individual woman who wants to breastfeed in the future.
