工作相關之肌肉骨骼不適(work-related musculoskeletal disorders, WMSD)是由於重複性的工作過度負荷,且經過長時間的累積造成肌肉骨骼或相關組織疲勞、發炎、損傷,其中因重複性作業促發肌肉骨骼傷害為最常見職業性疾病(勞動部,2014)。在醫療產業中,臺灣醫事放射師因工作需求常引起肌肉骨骼不適之症狀,本研究目的在探討臺灣南投地區醫事放射師肌肉骨骼不適盛行率與部位分佈之情況,更進一步探討工作特性對其肌肉骨骼不適的影響程度及其與生活品質間的關係。本研究使用北歐肌肉骨骼問卷調查表(Nordic musculoskeletal questionnaire, NMQ)及臺灣簡明版世界衛生組織生活品質問卷(WHO quality of life-BREF, WHOQOL-BREF)為研究工具來進行測量,並以臺灣南投地區執業之醫事放射師共102位為對象。資料收集後以SPSS V.22.0統計軟體進行描述性與包括卡方檢定(Chi-square)、費雪正確概率檢定(Fisher’s exact test)、獨立樣本t檢定(Independent Sample t-test)、Spearman correlation及 Pearson correlation相關檢定方式及迴歸(regression)等推論性統計分析,α 0.05,結果顯示醫事放射師WMSD盛行率高達80.4%,不適部位普遍都集中在肩膀、脖子、腰背及手或手腕等部位。腰背部位發生肌肉骨骼不適與研究對象所在的不同層級醫院,有顯著性差異(χ2= 3.89, p < .05)。操作乳房攝影/超音波是造成肢體部位不適的危險因子(OR=3.82)。而發生肌肉骨骼不適或不適總部位數,與生活品質總分或四項範疇皆成負相關。建議乳房攝影與超音波之受檢台高度符合人體工學;且排定班別避免同一天連續操作該儀器的時數過長,避免因長時間重複操作而導致肌肉骨骼不適發生,進而影響生活品質。
Work-related musculoskeletal disorders (WMSD) are due to repetitive work overload, and after a long period of accumulation, musculoskeletal or related tissues are fatigued, inflamed, and injured.Among them, musculoskeletal injury caused by repetitive work is the most common occupational disease (Ministry of Labor, 2014). In the medical industry, Taiwanese radiologic technologists often cause symptoms of musculoskeletal discomfort due to work needs. The purpose of this study is to explore the prevalence and distribution of musculoskeletal discomfort of radiologic technologist in Nantou, and to further explore the work characteristics of their muscles. The degree of influence of bone discomfort and its relationship with quality of life. The Nordic musculoskeletal questionnaire (NMQ) and the Taiwan Condensed World Health Organization Quality of Life Questionnaire (WHO quality of life-BREF, WHOQOL-BREF) were used as research tools for measurement. This study targeted a total of 102 radiologic technologists practicing in Nantou, Taiwan. After data collection, SPSS V.22.0 statistical software was used to describe and include Chi-square test, Fisher's exact test, Fisher's exact test, Independent Sample t-test, Spearman correlation and Pearson correlation test methods and regression and other inferential statistical analysis, α 0.05, the results show that radiologic technologists WMSD prevalence rate is as high as 80.4%, the discomfort is generally concentrated in the shoulders, neck, waist and hands or wrists and other parts. The musculoskeletal discomfort in the lower back position was significantly different from that of the hospitals at the different levels (χ2= 3.89, p <.05). Operation mammography/ultrasound is a risk factor for discomfort in the limbs (OR=3.82). The number of musculoskeletal discomforts or discomfort headquarters is negatively correlated with the total quality of life score or the four categories. It is recommended that the examination platform of breast photography and ultrasound be highly ergonomic; and schedule to avoid excessively long hours of continuous operation of the instrument on the same day, to avoid musculoskeletal discomfort due to repeated operations for a long time, and thus affect life quality.
