本研究目的在於評估音樂治療對心肌梗塞病人焦慮程度及生理反應之成效。自2011年01月至2011年12月採類實驗性研究設計以立意取樣方式,以台中某醫學中心成人心臟科加護病房為取樣地點,研究對象心肌梗塞病人,採兩組前後測之實驗設計,實驗組30位及對照組30位。實驗組病人接受41分鐘音樂聆聽,對照組病人則採常規照護。研究者以結構式問卷進行資料收集,並進行音樂介入及協助問卷填寫。測量工具共含四部分:病人背景資料、音樂喜好評估、焦慮視覺類比量表及生理測量儀器,研究資料利用SPSS for window 13.0套裝軟體進行描述性統計、卡方檢定、獨立樣本t檢定、配對t檢定、單因子變異數分析、簡單迴歸統計分析。 研究結果顯示,實驗組病患接受音樂治療後,焦慮感受視覺類比量表得分平均5.3降至2.8,p<0.001呈顯著下降,在生理指標方面,音樂介入能有效降低病人的心跳、呼吸、收縮壓、舒縮壓、平均動脈壓p<0.001,顯示心肌梗塞病人聆聽音樂的確能使個案達到減緩焦慮的功效。期望本研究結果,以提供臨床護理人員運用聆聽性音樂治療的方式減緩病人焦慮之參考,以達到人性化護理之目標。
The purpose of this study was to assess the effectiveness of music therapy on the anxiety and physiological responses for patients with myocardial infarction.The duration of research was from January 2010 to December 2010.Quasi-experimental design and purposive sampling were used in this study,for which the participants were recruited from an intensive care unit of a medical center in Taichung. The participants were randomly assigned to music intervention group(30 patients) or control group (30 patients). The music-group participants received 41- minute music intervention while the control-group participants received only routine nursing care. A structured questionnaire was used to collect data. The researcher performed the music intervention and help participants to complete the questionnaire. The demographic data of participants, music preference and Visual Analogue Scale of Anxiety (VAS) of participants were identified. And, the biological index and heart rate variability were recorded by biological signal recording system. Data were analyzed with SPSS 13.0 for Windows statistical package software. The descriptive, independent t test, Chi-square test, one-way ANOVA and simple regression analysis were computed. The results of this study found that after the music listening for myocardialInfarction patients did decline the level of anxiety which showing that the mean score of was decreased Visual Analogue Scale of Anxiety (VAS) from 5.3 to 2.8 and reduced patients heart rate, diastolic pressure, systolic pressure, and mean arterial pressure (p<0.001). It was concluded that music listening for myocardial Infarction patients did reduce the level of anxiety. Hopefully, the results of this study will be an important reference for clinical nursing staff. And, Hopefully, that the music therapy method may help facilitate to achieve the goal of nursing with humanized.
