音樂治療對於子宫全切除婦女焦慮程度改善之影響

摘要

本研究目的爲探討音樂介入對於子宫全切除婦女焦慮程度之影響。本研究採類實驗設計，樣本取自台灣南部某一區域教学醫院研究對象爲接受手術之患者，一组實驗组及一组對照组各爲30人，先收對照组再收實驗組。實驗組於手術前一晚，手術前2小時、手術後第2天給予音樂治療，而對照組給予平常的手術之護理。本研究之研究工具爲視覺焦慮量表及生理指標（呼吸、脈博、收縮壓、舒張壓），資料收集於手術前一晚，手術前2小時、手術後第2天以SPSS10.0版進行分析統計包含百分比、配對t、卡方檢定、獨立t、重複變異因子變異分析等統計方法進行分析及檢定。本研究結果後手術前一晚至手術後第2天，實驗組焦慮分數由5.00±1.82分降至2.13±1.00分而對照組由4.25±2.51分降至2.60±1.14分，以重複變異量呈現顯著性的差異(F=9.525, p=.004)，在生理指標方面顯示音樂之介入能有效降低舒張壓(F=8.838, p=.004)。結果顯示音樂介入能顯著降低子宫全切除婦女術手之焦慮程度及舒張壓。本研究結果可提供護理實務工作者對行婦科手術照顧之參考。

The goal of this study was to examine the effects of music therapy on anxiety of the women with total hysterectomy. Subjects were recruited from a hospital in southern Taiwan. A quasi-experiment design was applied and two groups with 30 patients each were selected. The experimental group received music therapy while the control group received regular operative nursing care. The experimental group received 3 music therapy sections, including the first one in the night before the surgery, the second one in 2 hours before the surgery, and the last one in two days after the surgery. The study tools included visual anxiety scale and physiological index (respiration, pulse, systolic pressure, diastolic pressure). Data were collected corresponding to the administration of treatments. The statistical methods included percentages, paired t test and repeated measure ANOVA. The experimental group reduced their anxiety score from 5.00±1.82 to 2.13±1.00, while the control group also reduced their anxiety score from 4.25±2.51 to 2.60±1.14. The repeated measures ANOVA showed F=9.525, p=.004. The findings also showed that the music intervention significantly reduced diastolic pressure levels over time. The findings of this study indicated that the experimental group had significantly lower anxiety and diastolic pressure than the control group had. The results of this study provide a reference about the effect of music therapy in reducing anxiety for women receiving surgery.
