護理長向下影響行為與護理人員工作投入之相關性研究

摘要


本研究的目的在探討護理人員知覺其護理長向下影響行為的運用現況及工作投入情形,並進一步探討其影響因素及相關性。以結構式問卷進行調查,使用「向下影響行為量表」及「工作投入量表」研究工具進行收集資料。研究樣本取自中部某醫療體系醫院之護理人員,以分層隨機抽樣方式,共發出550份問卷,有效回收問卷443份(有效問卷回收率為80.55%)。資料分析方法包含描述性統計、t檢定、單因子變異數分析、皮爾森相關分析及階層迴歸分析。結果顯示:(1)護理人員知覺護理長向下影響行為的使用頻率及有效性,均未達中等程度(使用頻率M=2.73,SD=0.421;有效性M=2.79,SD=0.487)。 (2)護理人員知覺護理長向下影響行為的使用頻率及有效性,會因婚姻狀況、工作科別、他院及單位服務年資、職稱、職級、行政或管理課程、院區的不同,而有顯著的差異(p<.05)。(3)整體工作投入情形屬於中等程度(M=3.29,SD=0.439),以認同工作重要性最高,其次是主動參與程度。(4)護理人員工作投入會因年齡、婚姻狀況、本院及單位服務年資、職級、行政或管理課程、院區的不同,而有顯著的差異(p<.05)。(5)護理人員工作投入的預測因子:年齡、婚姻狀況、本院及單位服務年資、職級、行政或管理課程;向下影響行為使用頻率「理性說明、友善讚美、建立交情」;向下影響行為有效性「理性說明、友善讚美、威脅利誘、建立交情、向下求助」構面,共可解釋15.6%變異量。根據研究結果,可提供護理主管了解護理長向下影響行為與護理人員工作投入的相關性,做為管理上之參考,以提昇護理人員的工作投入,進而提供優質的照護品質。

The purposes of the present study were to discuss with the Head nurse''s downward influence behaviors and job involvement of hospital nurses, and further the related factors and the relationship . Constructed questionnaires containing downward influence behaviors scale and Job Involvement scale were utilized to survey the subjects. The lamination randomized sampling was conducted at a medical system hospital in central Taiwan. Questionnaires were sent to 550workers in the nursing field, and 443 data were replied and valid for data process.(response rate 80.55%). These statistical methods were applied including descriptive statistics, Independent t test, one way ANOVA, Pearson correlation analysis and Hierarchical regression analysis . The results showed that:(1) The perceived frequency and effectiveness of head nurse''s downward influence behaviors among staff nurses were less than the medium-degree (frequency M=2.73, SD=0.421;effectiveness M=2.79, SD=0.487). (2)The staff nurses'' perceived frequency and effectiveness of head nurse''s downward influence behaviors were significantly associated with marital status ,departments, working years in other hospital, working years in the unit, job titles, clinical ladder levels, administration programs and different hospitals(p<.05). (3)The job involvement of the staff nurses is medium-degree ,which is the highest for importance of job and followed by initiative participation (4)The job involvement were significantly associated with age, marital status , working years in the hospital, working years in the unit, clinical ladder levels, and administration programs and different hospital (p<.05).(5) The major predictors of the job involvement included age, marital status, working years in the hospital, working years in the unit, clinical ladder levels, and administration programs ; nurses'' perceived frequency in rationality,friendliness, invoke role relationship and perceived effectiveness in rationality,friendliness,threaten,invoke role relationship,ask of downward influence behaviors. The related variables explained 15.6%. The finding of this study could be used as a reference for clinical nursing manager to understand about the relationship between Head nurse’s downward influence behaviors and job involvement in the nurses. It may help managers to promote the nurse’s job involvement and improve the quality of nursing care .
