護理人員工作壓力、家庭支持及留任意願之研究

摘要


全民健保使臺灣醫療保險普及化,不斷的增加服務量,臨床照護工作更趨複雜。臨床上之護理人員,須身兼數職,承受著工作帶來身心煎熬,在有限的能力及時間,於工作中引發許多負面效應,於是延生出護理人員的醫療服務品質、繼續留任及人力缺乏等問題,對醫療服務品質的維持是一大挑戰。因此本研究目的想探討影響臨床護理人員之工作壓力問題,瞭解其家庭支持度的狀況及留任意願之相關性。 本研究以問卷調查法為主,於中部某教學醫院護理人員進行全面普查研究,以研究者所編製的「工作壓力家庭支持留任意願」量表為工具,共發出調查問卷262份,有效回收問卷244份,有效回收率93.1%。採用的統計方法是SPSS 17.0統計套裝軟體進行敘述性統計、獨立樣本t檢定、單因子變異數分析、Pearson相關分析及多元逐步回歸統計方法進行問卷資料分析。 研究結果顯示,護理人員在角色衝突(p < .001)與專業知能(p < .05)的壓力感受越高時,則留任念頭的程度則會越低;實質性支持(p < .01)顯示護理人員在實質性支持越高時,則留任念頭的程度越高。工作壓力與家庭支持各分層面,對整體留任意願的預測力大小依序為角色衝突、實質性支持與專業知能,其中以角色衝突最具預測力。 關鍵字:工作壓力、家庭支持、留任意願

Healthcare insurance was popularized by The National Health Insurance in Taiwan, thereby increasing and complicating the workloads of clinical nurses. Such nurses are responsible for multiple tasks and are burdened with mental and physical stress caused by their work, which under limited capacity and time, may present various negative effects in these nurses’ work. This leads to problems related to nursing quality, intention to stay, and labor shortage, and therefore presents a great challenge to maintaining medical service quality.. Therefore, the purpose of this study was to explore factors affecting the work stress of clinical nurses and examine the relationships of work stress with family support and intention to stay. The questionnaire survey method was adopted to conduct a general census on nurses in a teaching hospital in central Taiwan. The self-developed “work stress–family support–intention to stay scale” was used as the survey instrument, with 262 questionnaires distributed and 244 valid responses returned (valid return rate = 93.1%). SPSS 17.0 Descriptive Statistics, Independent Sample T- Test, One-way ANOVA, Pearson Correlation and Multiple Stepwise Regression Analysis were applied to analyze the data. The results showed that nurses who perceived high stress levels from role conflicts (p < .001) and professional knowledge (p < .05) demonstrated a low intention to stay, whereas those who perceived a high level of substantive support (p < .01) exhibited a high intention to stay. Regarding the various dimensions of work stress and family support, role conflicts had the highest predictive power, followed by substantive support, and professional knowledge.
