摘要


電腦資訊的普遍化,無紙化成為企業界努力的目標,對醫界而言,行政文書的無紙化與企業界的做法類似,但是在醫療資訊方面,則因醫療儀器的製造技術、設備汰舊換新經費的龐大、資訊系統整合等問題,使得許多醫院在進行醫療資訊無紙化的時候,顯得相當猶豫。 本研究係以台灣中部某醫學中心級醫院為例,探討該醫院如何以比較經濟、安全有效率的方式及短短6個月的時間內,導入影像儲存及傳輸系統(Picture archiving and communication system, PACS),供其他有意實施的醫院做為參考。 經過探討的結果,顯示該醫院導入PACS具有成功的特色:(1)導入前規劃者與使用者充分的評估與溝通,並分析使用端的分佈及預估流量,作為網路段架設的依據,(2)採用分散式架構,依使用者之特性將網路傳輸負荷妥善分配,(3)HIS與PACS的網路平常分開,當有需要時才透過core switch抓取PACS的資料,(4)資料有3至4份備份,並且儲存在不同建築物及不同地區,(5)影像一產生立即分配到各負責判讀醫師工作站,提高出報告的時效。

Since the fast development of computer science, just 10 years ago, paperless policy became the goal of the enterprises. In the hospitals, paperless policy was also executed by administration departments. But for the medical information and images’ filmless are just begin in Taiwan in the recent year. How to integrate hospital information systems(HIS)and pictures archiving and communication systems (PACS)to be efficiency and safety are still the primary cared issues. The purpose of this paper is to present the experience of implementing a HIS-PACS integrating at China Medical College Hospital, which is a medical center in Taiwan. We found that in this hospital, there are some successful features as followed:(1) Information department have adequately discussed with medical departments to decide the distribution of clients, and estimate data stream before PACS induced. (2) A high performance distributed database system is adopted according to the designed response time. (3) PACS network is separated from HIS network and communicate by core switch to avoid influence of the communication speed of HIS. (4) 3 to 4 backup databank to store in different sites. (5) The images are transferred immediately to the responsible doctor to make report as it produced.
