造釉細胞瘤：由免疫組織化學檢查來探討其治療方式

摘要

在手術治療上之多樣化，對於從事口腔顎面外科之醫生而言，實具挑戰性，除了組織病理學上之檢查與放射線（包含電腦斷層與核磁共振）檢查外，以免疫組織化學法觀察此腫瘤上皮細胞，使我們更加了解此腫瘤之生物行為，此種行為往往能提供我們在做鑑別診斷與手術方式之參考，雖然國外在此方面之研究報告不多，我們回顧了過去的文獻，討論了各個免疫組織化學表現，如以抗體分別檢查了不同之抗原如EGF或EGFR、PCNA、Ki-67、keratin等，以及lectin組織化學也包含在內，我們將之綜合歸納，提出了初步的看法，也期待國内能有更多此方面之病例研究，以瞭解此造釉細胞瘤之特性。

It is challenging for us oral surgeons to manage the ameloblastoma properly because of the multiplicity of its surgical treatment. Besides the histopathological and radiological (including CT & MRI) examinations, the immunohistochemical examination of the tumor epithelial cells provides us further understanding of its biologic behavior. The advances in the understanding of its behavior may help us make accurate differential diagnosis and select the treatment modality properly and definitively. We reviewed the literatures regarding this subject and discussed various immunohistochemical expressions against antigens such as EGF or EGFR, PCNA, Ki-67, Keratin etc, and also, lectin histochemistry was included. The important points were summarized although the reports were not voluminous. We expect more immunohistochemical studies done in the near future in this country. It may shed further light on this subject and unravel the mystery of this tumor.
