The Effects of Diabetes on the Bone Formation Around Implants of Rats

糖尿病對種植于老鼠之人工植體骨生成的影響

摘要


當病人因外傷、牙週病或腫瘤等因素造成部分或全部牙齒喪失時,傳統的牙科贗復方法不見得能滿足所有的牙科患者。此時,牙科植體便提供病患另一種治療方式,依照以往的文獻報告,人工植體應用於正常病患有極高的成功率,然而人工植體應用於糖尿病患者仍備受爭議。因糖尿病容易導致骨骼癒合不良及傷口感染。此實驗之目的及應用實驗性糖尿病動物,探討人工植物應用於糖尿病動物的可行性。結果顯示正常鼠之植體周圍有大量的板狀骨(lamellar bone)及成熟骨(mature bone),而糖尿病鼠植體卻充滿了纖維組織(fibrotic tissue)且骨頭多爲綿狀骨(woven bone)及組織零亂(disorganized)之組織。由此實驗得知在使用人工植體於糖尿病患者時,需特別小心,因控制不良的糖尿病,其人工植體周圍之組織多爲不成熟之結締組織,而非如正常鼠之植體周圍有大量的板狀骨,因此易造成人工植體之失敗。

Several factors are known to influence the anchorage of an implant in bone: biocompatibility of the metal, design, surface structure, surgical technique, biomechanical factors, factors affect bone formation and remodeling. To investigate the effects of diabetes on the osseous healing around the implant, experimental diabetic animals model was used. Implant was inserted into the tooth extraction sockets immediately after maxillary molars were extracted in both normal and diabetic rats. The results revealed that there were a large amount of new bone formation while only immature and disorganized fibrous tissue was found around the implant of the diabetic animals. Future study is necessary to quantify the bone around the implants.
