摘要


血液透析用水及透析液的品質對透析病人非常重要,如果血液透析用液內毒素(endotoxin)含量過高,會造成病人發熱等反應,但傳統的細菌培養無法反應出內毒素的含量。因此本文介紹三種鱟變形細胞溶出物(LAL)內毒素檢驗方法,包括凝膠法(gel clot technique)、動力濁度法(kinetic turbidimetric assay)、動力呈色法(kinetic chromogenic assay)的原理及方法。本透析中心為提供血液透析病人更好的透析品質,由2013年開始進行RO水的內毒素檢測。本文也提供本中心這兩年來的檢驗內毒素經驗。

The quality of water used for dialysis is very essential for hemodialysis patients. Endotoxin in dialysate will cause pyrogenic reactions among dialysis patients. However, traditional methods with bacterial culture cannot reflect the concentration of endotoxin properly. This article introduced the principle and methods using Limulus Amebocyte Lysate (LAL) assay to examine endotoxin, including Gel Clot Technique, Kinetic Turbidimetric Assay, and Kinetic Chromogenic Assay. For providing better dialysis quality, this dialysis center had measured endotoxins in reverse osmosis water using Kinetic Turbidimetric Assay since 2013. This manuscript provides our experience in these two years.
