血液透析患者衰弱評估及影響因子探討

Factors Associated with Frailty in Hemodialysis Patients

摘要


衰弱(frailty)是患者虛弱及依賴性增加之臨床現狀,血液透析患者為衰弱的高危險群。本研究以台中市某診所慢性血液透析病人為研究對象,參考Fried等人訂定之指標進行衰弱評估。結果顯示,血液透析患者(n = 72)衰弱前期及衰弱的盛行率分別為33%及22%,其中女性患者衰弱盛行率較男性高,無衰弱組之白蛋白及血色素較衰弱組高。多變項邏輯斯迴歸(multivariate logistic regression)分析發現,女性、高齡、低白蛋白及運鐵蛋白飽和度為衰弱顯著之預測因子。本研究旨在探討血液透析患者可行之衰弱評估及預測指標,以及早評估衰弱狀況。

Frailty is defined as a clinically recognizable state of weakness and loss of independence. Patients receiving maintenance hemodialysis are at an increased risk for frailty. A cross-sectional study focusing on frailty in hemodialysis patients (n = 72) was conducted at a clinic in Taichung. The diagnosis of frailty was based upon the five dimensions of assessment as proposed by Fried et al in this study. The prevalence of pre-frailty and frailty among patients on hemodialysis was 33% and 22%, respectively. Overall, there was a stronger correlation of frailty in female patients versus male patients. Additionally, serum albumin, hemoglobin, and transferrin saturation levels were significantly higher in non-frail groups. Multivariate logistic regression analyses demonstrated that patients who were female, at older age, lower serum albumin levels, and transferrin saturation were significantly more associated with frailty. Early identification of frailty and its risk factors are important and practical in the treatment of chronic hemodialysis patients.
