某區域醫院孕婦牙周狀況與治療需求之調查研究

Periodontal Conditions and Treatment Needs of Pregnant Women of a Regional Hospital

摘要


為了瞭解孕婦牙周疾病的盛行率(prevalence)與嚴重程度(severity)及懷孕月數、孕婦年齡、懷孕胎次與牙周疾病間的關聯,並進一步探討孕婦的牙周治療需求,本研究以台安醫院婦產科567位門診產撿孕婦為研究對象,根據其社區牙周治療需求指數(CPITN)作一初步調查研究。本研究結果顯示,受檢孕婦的牙周疾病盛行率高速100%;其中40.39%的孕婦有≥6mm的牙周囊袋,49 91%的孕婦有4-5mm的牙周囊袋,8.64%的孕婦僅有牙結石,1.06%的孕婦僅有牙齦探測出血(bleeding on probing)的現象。本研究中牙周疾病的盛行和嚴重程度與受檢孕婦的年齡層及胎次無顯著正相關。受檢族群中,懷孕第八、九個月的孕婦其有≥6mm的牙周囊袋的比例明顯增加。牙周治療需求方面,100%的孕婦均需加強口腔衛教;其中98.94%的孕婦需再加上結石刮除,40.39%的孕婦除了衛教、結石刮除之外還需要加上複雜的牙周治療。

Changes in gingival condition during pregnancy have been documented. The prevalence of this change in gingiva ranges from 35% to 100%, but there is no available data about periodontal condition changes during pregnancy in Taiwan. The periodontal conditions of 567 pregnant women of Taiwan Adventist Hospital (TAH) were assessed according to the CPITN (community periodontal index of treatment needs).The purposes of this study were to evaluate (1) the prevalence and severity of periodontal condition changes during pregnancy, (2) the correlation between gestation age; different age group of pregnant women; the birth order and the periodontal conditions, and (3) the periodontal treatment needs of pregnant women. The results of CPITN screening test in this study show that 100% examined pregnant women had periodontal disease; the percentage of pregnant women with bleeding on probing (code 1) was 1.06%, 8.64% screened pregnant women had calculus or other plaque retentive factors(code 2), 49 .91% surveyed pregnant women had 4 or 5 mm pockets(code 3), 40 .39% pregnant women in this study had ≥ 6 mm pockets(code 4). The percentage of pregnant women with ≥ 6 mm pockets was significantly higher in the 8- and 9- month group. There is no significant correlation between different age groups and the periodontal conditions, so as to birth order is not correlated with the periodontal conditions. All pregnant women in this study need oral hygiene instruction (OHI)(TN1), 98.9% examined pregnant women need OHI and prophylaxis (TN2), 40.3% screened pregnant women need OHI and prophylaxis and further complex periodontal treatments (TN3). These findings suggest that a special program of periodontal disease prevention for pregnant women is recommanded.
