引導組織再生術治療上顎第一大臼齒之第三級根叉侵犯－病例報告

Guided Tissue Regeneration Treatment in Class III Furcation Involvement of Maxilary First Molar-A Case Report

摘要

目前臨床上使用引導組織再生術治療大臼齒第二級根叉侵犯的成功率較高，而對第三級根叉侵犯的治療效果其可預測性較低，特別是上顎臼齒因有三個根叉，所以困難度更高。本篇報告使用Gore- tex牙周再生膜合併冷凍乾燥脫鈣異類皮質骨治療上顎第一大臼齒第三級根叉侵犯，術後五星期將再生膜移除後可見近心及頰側根叉區完全被新生牙周組織覆蓋，一年半後牙周探測結果改善良好且X光片可見明顯的骨質密度增加，目前追蹤二年半，情況穩定。

Using guided tissue regeneration for treatment of molars class II furcation involvement is more predictable, but less predictable for class III furcation involvement, especially it is more difficult for maxillary class III furcation involvement due to three furcations. This case report is using Gore-tex periodontal membrane combined with demineralized freeze dried bone allograft for the treatment of maxillay first molar class III furcation invo lvement. Buccal and mesial furcation defects was completely covered with new periodontal tissue when the membrane was removed after five weeks of GTR. The improvement of periodontal probing data and increasing bone density by X-ray film were found after one year and six months. The condition is stab le after 2 years and six months follow-up.
