發生於根管治療後的牙齒與植牙的牽連痛：病例報告

Referred Pain on the Root Canal Treated Tooth and Dental Implant: A Case Report

摘要

中年女性患者抱怨在左側下顎第二小臼齒接受過牙髓治療的牙齒以及左側下顎第一第二大臼齒植牙區域有疼痛情形。臨床上及放射線學的檢查皆在正常範圍之內。經過會診贗復科及牙髓病科，並無特別發現，建議暫時觀察。門診追蹤時拍攝同側上顎第三大臼齒X光片後，發現於該第三大臼齒上有一放射線性透過之病兆，懷疑是牙髓炎所造成的下顎後牙區的疼痛，因此建議將該牙齒拔除。經過拔除左側上顎大三大臼齒後，原本位於接受過牙髓治療的左側下顎第二小臼齒以及左側下顎第一第二大臼齒植牙區域之疼痛情形完全消失並且追蹤一年未再復發。籍由此病例報告之特殊情形，提供大家在日常治療過程中常忽略之另一觀點。

A middle age female patient complained of pain over the root canal treated lower left second premolar and molar implant area. Clinical and radiographic examination did not reveal any sgnificant change. There was not any conclusion of the complained pain from endodontic and prosthodontic consultation. Then we suggested her to close observation, Six months later, she visited us again arid took radiography on upper left molar area. There was a big radiolucency on the upper left third molar. That tooth was then removed immediately. The previous complained pain on the lower left premolar and molar implant area was gone The pain did not happen for one year follow-up after third molar extraction Therefore referred pain was diagnosed. Through this unusual case, it may provide another thought for our daily practice. The definition of referred pain and related literature were also discussed.
