上顎埋伏犬牙的矯正治療

Orthodontic Treatment of Maxillary Impacted Canines

摘要

在矯正治療中，會有機會到上顎埋伏犬牙的病例；對這些埋伏牙，最好的治療方式，應該是以手術暴露，再以矯正拉出至正常位置。本文就十二個上顎埋伏犬牙的治療例，描述及討論手術暴露，所遇到的問題；並建議對於唇側位置的埋伏犬牙，儘可能用根尖移位瓣膜手術，同時暴露埋伏牙及提供附著性牙齦於埋伏牙上的露出性萌牙(open erupiton)方法；對於顎側位置的埋伏犬牙，（包括異位生長ectopic 及垂直位置生長的埋伏犬牙），則用覆蓋性萌牙(closed eruption)方法，以矯正裝置拉出後，再考慮附著性牙齦問題。而對於附著埋伏牙上的矯正裝置，仍以直妾粘著(direct bonding)為主；錨固定(anchorage preparation)可用固定方式(fixed type)或活動裝置(removable appliance )為之。

”It is not uncommon to find the impacted maxillary canines during routine orthodontic
examination. Unless we create the space in the arch and the tooth will erupt into the
arch by itself, surgically uncover the tooth and orthodontically extrude it thereafter will
be the first choic of treatment. Tweleve maxillary impacted canines,
including six labially positioned teeth and six palatally positioned teeth were treated. In the
labially impacted teeth, we chose an ””open eruption”” method, using a repositioned
mucoperiosteal flap (apically positioned flap or lateral pedicle flap) to uncover the tooth
and provide the keratinized gingiva on it. In the palatally impacted canines, a ””closed
eruption”” method was chosed. A simple direct bonding technique was used for orthodontic
traction. Several articles were reviewed and some considerations about the surgical techniques and fixation of the attachment for impactions are discussed.”
