摘要

多型性腺瘤（pleomorphic adenoma）為唾液腺最常見的良性腫瘤，但是偶爾也會發生惡性變化，甚至遠處轉移，尤其是那些長期存在或術後復發的病例。這種情形稱為多型性腺腫中的癌（carcinoma ex pleomorphic adenoma）。本文報告一例79歲女性，注意到下頜部腫塊6年，經手術切除證實為下頜下腺惡性腫瘤。由於下頜的巨大腫瘤較少見，除了提出此例的臨床過程及治療經過外，並根據相關文獻予以討論。

Pleomorphic adenoma is the most common benign salivary toumor; however, malignant transformation and distant metastases can be found in some of the long persisting or recurrent cases. We report a 79-year-old woman with a giant submandibular mass for over 6 years. She came to our hospital because or rapid growth of the tumor accompanied by pain over the previous 6 months. After surgical removal, the pathology revealed focus of malignant transformation in the pleomorphic adenoma. She was discharged uneventfully, but local recurrence was suspected 2 years later. Pain and rapid growth are the warning symptoms of the pleomorphic adenoma with malignant change. Surgery is the mainstay of treatment. For early detection of recurrence, a long-term regular follow-up is recommended.
