Sebaceous carcinoma is an uncommon head and neck malignancy. Usually, the lesion arises in the meibomian glands of the eyelid and is a malignancy with a high potential of head and neck metastasis. Diagnosis is difficult because of its low incidence and deceptive clinical presentation. Treatment requires wide surgical excision with neck dissection and post-operative irra-diation. Since 1983, we have had three patients with eyelid sebaceous carcinoma and neck masses in our department. Neck dissection was performed for the first and third patients and parotidectomy for the second. Pathological examination all revealed metastatic sebaceous carci-noma. The firs two patients then received post-operative irradiation. In spite of vigorous treatment, the first patient died of multiple bone metas-tases two years later and the last patient showed brain metastasis three months later, only the second showed good result. It is a malignancy with poor prognosis therefore requires clinical alert for early detection and management.
