Laryngeal melanoma is a rare disease, Only 52 cases have been reported in the literature. It is usually found in Caucasian males. Among the cases described, only 10 showed “in situ change”, which is an important criterion for diagnosis of primary melanoma. We present a Taiwanese woman with primary laryngeal melanoma. The patient demonstrated a black polypoid tumor in the supraglottic region nearly totally obstructing the larynx. Endoscopic biopsy showed malignant melanoma. The patient underwent total laryngec-tomy and bilateral modified neck dissection. Microscopically, “in situ change” was defi-nitely noted. No local recurrence was noted, but lung and liver metastasis were proven 5 months later. The patient finally died of disseminated disease. Clinicians need to be aware of the clinical manifestations of primary laryngeal melanoma, as it is quite rare in Asians.
