摘要
平滑肌瘤是一種源自平滑肌細胞的良性腫瘤因為喉部平滑肌較少，所以喉平滑肌瘤在臨床上就很少見。台北長庚醫院耳鼻喉科經歷一男性患者，主訴過去1年來一直有喉異物感，最近3天因為喉嚨刺痛，漸漸感到聲音沙啞，呼吸困難，被家人送至急診。經間接喉鏡檢查，發現聲門下區有一腫瘤。頸部側面放射線攝影發現在第6、第7頸椎的相對位置，有一圓球陰影。緊急施以氣管切開後，呼吸窘迫現象立刻緩解。在全身麻醉下，經顯微雷射手術摘除腫瘤，病理報告證實為罕見的血管性平滑肌瘤。經21個月追蹤檢查，並無復發跡象。由本病例，作者建議對於聲門下區的良性腫瘤，可以考慮在術前作好氣管造口術，在全身麻醉之下，經口作喉內視鏡顯微雷射手術將腫瘤切除。

Leiomyoma is a benign tumor of smooth muscle origin and is uncommon in the larynx. We experienced a 44-year old male patient with the chief complaint of lump sensation in the throat for one year, as well as hoarseness, haryngeal pain and progressive difficulty in breathing for 3 days. Because of sudden onset of dyspnea, he was sent to our emergency department. Indirect laryngoscopc examination disclosed a subglottic tumor. X-ray for the soft tissue of the neck revealed a spherical opacity opposite to the C6 and C7 vertebrae. Under general anesthesia, the entire mass was extirpated with CO2 laser endoscopically, and the pathologic diagnosis was a vascular leiomyoma. There is no evidence of recurrence after twenty-one months. We suggest that preliminary tracheostomy and surgical extirpation with CO2 laser via endoscopy can be a reliable choice of treatment.
