Inferior Turbinate Reduction Surgery Using KTP Laser

摘要


背景:KTP雷射因止血能力佳、在組織內穿透能力強,且導光器操作靈活,因此成為鼻內手術的輔助工具。本研究主要目的是探討KTP雷射實施下鼻甲手術的成效。 方法:本研究以回溯性的方法探討了33例慢性肥厚性鼻炎的患者,在接受KTP雷射下鼻甲縮小手術後的結果,其中男性19例,女性4例,平均年齡36.9歲,平均追蹤時間為12個月。 結果:研究發現KTP雷射對於解除慢性肥厚性鼻炎所造成的鼻塞症狀效果良好,而對於改善鼻涕倒流症狀效果較差,同時研究也發現KTP雷射下鼻甲縮小手術頗適合以門診手術方式進行,術後出血的機率不高,惟手術後需細心的局部治療,以防止沾黏的情況發生。 結論: KTP雷射下鼻甲縮小手術之效果頗令人滿意,且產生併發症的機率不高。如果術者有足夠之經驗,KTP雷射是一個實施下鼻甲縮小手術不錯的工具。

BACKGROUND: The KTP laser (potassium-titanyl-phosphate laser) is a useful tool for intranasal surgery, owing to its excellent hemostasis and tissue penetration capabilities. The maneuverability of its handpiece is also a clear advantage over other types of laser, to facilitate rhinosinal surgery. The aim of this study was to evaluate the usefulness of the KTP laser for inferior turbinate surgery. METHODS AND MATERIAL: Thirty three patients (M:F 19:14, mean age 36.9 years)with chronic hypertrophic rhinitis underwent KTP laser inferior turbinate reduction surgery. The mean follow-up period was 12 months. RESULTS: The KTP laser effectively relieved symptoms of nasal obstruction caused by inferior turbinate hypertrophy. The outcome was less ideal in terms of reducing post-nasal drip. The incidence of post-operative bleeding was low. Meticulous local treatment was necessary to prevent synechia formation. CONCLUSION: The results of inferior turbinate reduction surgery using the KTP laser were satisfactory and complication rates were low. In experienced hands, the KTP laser should be an useful tool for inferior turbinate surgery.
