An aberrant internal carotid artery in the middle ear is a rare otological finding. This anomaly may result in serious hemorrhage if a tympanotomy is performed or during middle ear manipulation if the otologist is unaware. A 20-year-old lady is described, who presented complaining of right sided pulsatile tinnitus. An intratympanic reddish mass was noted in the right ear and was confirmed by high resolution CT of the temporal bone to be an aberrant intratympanic internal carotid artery and no attempt was to correct the anomaly. The patient was given advice about the condition and cautioned against the risks of manipulations including biopsy or paracentesis on the middle ear.
