探討踝關節損傷的簡便式固定及其應用

Study and Application for Ankle Joint Injury in Simlping Fixation

摘要


常見的踝關節損傷與踝關節具有三項受力特點有關。如何重視受傷的踝關節其受力穩定度是治療踝關節損傷的關鍵。踝關節骨折與扭、挫傷皆需同時考慮固定與活動等問題。病例報告:一位64歲男性因車禍致右內踝部骨折,以簡便式固定,達到消除腫痛、穩定癒合之療效。討論:固定依符合人體生物力學原理,並支持患者進行早期功能鍛煉,則固定方法和材料不必拘泥。筆者以處置踝部骨折病例之模式應用於踝關節急性挫傷,可呈現早期加強支持的療效。結論:簡便式固定應用於踝或其他關節扭、挫傷併患者不宜積極活動,須加以固定者,或應用於懷疑線性骨折而暫時不能確診,須加以固定者,其療效值得進一步的研究、探討。

The biomechanism of common ankle injury involves three stress vectors in ankle. Focusing stability of ankle joint is the key of treating ankle injury. Sprain, contusion, and fracture of ankle all need to take into the correlation between fixation and mobility. Case Report: A 64-year-old male had the right ankle fracture due to traffic accident. He was received a simpling fixation of right ankle, and his restoration was smooth. Discussion: According to the fixation with biomechanical principle and the plan of early functional training with support, we should free choose methods and materials of fixation. We applied the treatment mode of ankle fracture to acute ankle injury and the result showed the efficacy of early enhancing support of the ankle joint. Conclusion: It is worth further study that the simpling fixation can be applied to patients with sprain, contusion or linear fracture suspected of ankle or other joints having contraindication of active moving and having to be fixed.
