A Giant Urethral Caruncle Presented with Acute Urinary Retention - A Case Report and Review of Literature

一巨大尿道肉阜以急性尿滯留表現－病例報告與文獻回顧

摘要

尿道肉阜大部份好發於停經後的婦女，多屬於良性腫瘤；其大小甚少超過一公分，且好發於尿道後緣(posterior lip)。尿道肉阜引起之症狀少有以急性尿滯留為表現。本文報告一68歲女性病例之巨大尿道肉阜，其大小超過三公分；並且引起急性尿滯留，至本院急診求治。當時因無法找到尿道口，所以緊急作恥骨上膀胱造口術引流尿液約800毫升；其後病人住院，接受手術切除尿道肉阜後順利出院。

Urethral caruncles are the most frequent benign tumors of the female urethra and are sel-dom greater than 1 com in diameter. These are usually found in the menopausal period and in the posterior lip of the urethral meatus. Symptomatic urethral caruncle presenting as acute urinary retention is quite rare. We reported a 68-year-old female patient with acute urinary retention due to a giant urethral caruncle which was difficult to identify the urethral meatus by catheter insertion at our emergency room. Therefore, emergent suprapubic cystostomy was performed and 800 ml urine was drained. Wedge resection of the lesion providing complete excision was performed and postoperative priod was smooth without any adverse sequelae. (J Urol R.O.C., 8:206-209,1997)
