Cholesterol granuloma of the frontal sinus is a rare disease which has the characteristic pathology including granulomatous reaction surrounding cholesterol crystals and altered blood. Hemorrhage, disordered ventilation and impaired drainage are decisive pathogenic factors in the causation of cholesterol granuloma. Cholesterol granuloma of the frontal sinus is treated effectively through radical clearance or drainage of the sinus cavity. A review of the literature on cholesterol granuloma suggests that the early clinical manifestations and radiographic features, ie., high-resolution CT, MRI, allow for a critical preoperative differentiation. We present a case of huge cholesterol granuloma of the frontal sinus following a falling-down accident 10 years previously. The patient was treated by tumor extirpation through osteoplastic approach to the frontal sinus and fat obliteration with a good result.
