眩暈的內科治療之評估

摘要

由於眩暈是一種非常主觀的感受，截至目前為止，尚無一可靠的方法用來評估其治療的效果。大體說來，如果病人自覺眩暈袪除，即可評定治療有效，但此法卻失之主觀，不具科學性。本研究以病人主觀上自覺眩暈的有無，與客觀上醫師們判斷自發或誘發眼振之有無來作為藥物療效評估之根據。如果經藥物治療3個月後，眩暈及眼振二者皆消失時，則評為有效，治療成功。根據此一法則，利用Buflomedil加上神經賦活劑與精神安定劑治療30名眩暈病人的結果，70%評為有效。

Since vertigo is a very subjective feeling, there is no reliable parameter to evaluate the outcome of its treatment up to date. It seems non-scientific to assume that a treatment giving subjective relief of vertigo, felt by the patient himself, can be judged as effective. In this study, both subjective feeling of vertigo and objective detection of spontaneous and/or induced nystagmus (head shaking nystagmus), were used to evaluate the efficacy of medical treatment for vertigo. If there was neither subjective vertigo complained of by the patient nor objective nystagmus observed by the doctor after 3 months of antivertigo treatment, the outcome was classified as effective. Based on this assumption, medical treatment using Buflomedil, a vasoactive agent, for 30 patients with vertigo, there was an effect rate of 70%.
