Castleman's Disease in the Neck － Case Report
摘要
Castleman氏病（Castleman’s disease），亦即巨大淋巴結增殖（giant lymph node hyperplasia），乃是淋巴結增生的一種特殊型式。組織學上有兩個明顯的次型-透明血管型（hyaline-vascular form）和漿細胞型（plasma cell form），大部份病例是以無症的縱隔腔腫塊來表現，只有少數病例單獨以頸部腫塊來表現。雖然它的病因仍未清楚，但手術切除一般都可以治癒。本院於民國81年元月間經歷一頸部單獨巨大淋巴結增殖的病例，經手術切除後，追蹤至今並無復發現象。

Castleman’s disease, i.e. giant lymph node hyperplasia, is a benign lymphoproliferative disorder. There are two distinct histologic subtypes － hyaline-vascular type and plasma-cell type. The hyaline-vascular type usually presents with localized form and the latter is often associated with systemic manifestations. Most patients are asymptomatic and under age 30, and a mediastinal or hilar mass is found incidentally on chest radiograph. The pathogenesis is unkown. Complete surgical excision is the treatment of choice and is generally curative. It can be difficult for the clinician to diagnose Castleman’s disease when cervical lymphadenopathy is the only manifestation. A case of giant lymph node hyperplasia presenting as a lump in the neck is reported together with a review of the literature. The recognition of an occasional variant of this disorder with a potential malignant course and its association with AIDS call for a greater awareness of this disease among all clinicians.
